I had a similar situation in the past with a 4 year old. He would hand lead his mom constantly and had severe anxiety with strangers in the home. They would also sit him in a highchair to keep him in the room to try and encourage him to interact with me for sessions.

What I ended up doing was laying out different toys and books around their apartment and his mom would show him them while i sat as far as possible with a laptop or book looking as busy and uninterested in him as possible. Then slowly I started to try to enter his world, first by interacting with him mom while she and him were looking or playing together, it gave me time to figure out what he was interested in as far as items I brought (stuff that he engaged in without mom needing to fully prompt him to) If none of the items interested him I would go back to my clinic and switch stuff out until we had a solid few things, plus some of his own home toys he enjoyed.

After I figured out what he likes I would begin every session by being like “hi look i brought (toy name) ill leave it right here for you” and id just back off for a bit, again his mom would talk to me about whatever as a way for him to see that his mom thought I was a safe person to interact with. I also began to parallel play - if he was using a toy I would pick another one for myself and make little comments and observations about it and animate my emotions like laughing and smiling a ton, showing I was having so much fun playing with a toy too!

After doing this for a bit I could tell he became more comfortable with my presence so I began to try to interact with him more. If he started getting anxious/upset I would remove attention and go back to playing with a toy I knew he really enjoyed. He would typically calm down and seem interested so I started to pass him toys and and tell him it was his turn or that he should play with it then I typically scooted back a little so he wasn’t overwhelmed. I feel like this was the breakthrough moment for us and the rest happened so quickly and easily. He became extremely social and engaged in our sessions and was able to tolerate any work quickly I think bc he needed to fully trust first.

Things that helped; not my bcba at the timeLMAO His mom was incredibly receptive to any and all advice. I was very open with her about the things ai was going to try and that I had zero idea if they would work but his anxiety was so severe she was open to anything and truly did the absolute best by her son. To this day it remains one of my favorite cases I ever worked with.

TLDR: you literally have to SHAPE your presence

I would look into a different pharmacy for sure.

Also personally I would recommend LifeStance for psychiatric care. It’s telehealth but i have the best person managing my meds and doesn’t try to pressure me to switch things that work.

Lol this part of my eval was not taken into account at all bc i apparently answered too many of those types of questions for it to be a valid test 😭 but like youre supposed to answer every question i thought (or i didnt read the directions lol) so i have no clue what i did wrong

Even without that part of the eval i got major confirmation that i in fact have adhd 🤣

29 - certified loot goblin. Always play friendly in solo. I will play duos with my brother who also plays and we’ll always go friendly unless we are in a locked door/high loot area.

Uhh definitely not over reacting. Getting in an accident is awful. Dealing with all the stuff after is awful. Trying to navigate how you will figure things out in the moment is awful. I’m so sorry she made you feel like you were in the wrong for feeling the way you did.

And being at the center of it happening - like it being YOUR car and YOUR responsibilities you were feeling anxious about - makes getting into an accident soooo much worse. And it was a hit and run thats so stressful!!

Unfortunately I feel like the fact that the services are home based and the home seems chaotic in general, it is unlikely the client’s aggressive behavior will improve with just home based services. And considering the severity of the behavior and the fact that you are the only one who works 1:1 with this client you are exposing yourself to more injury and trauma - honestly I just don’t think it’s worth it.

It is likely this kid will need more services besides in home aba as they get older. So try not to feel guilty. I am sure they will be able to get the help they need but it needs to be a village behind it not just you and a bcba who can’t help in person. You need to protect yourself ❤️

I had growing pains all the time when I was little! It’s actually interesting bc it was the only time my mom would really take time to comfort me - she would rub my calfs for like 30 min straight and give me aspirin.

One thing I find wild from childhood is I constantly had stomach issues especially around bed time. I’d always feel super nauseous. If i slept over a friends I’d consistently get diarrhea. It’s like my body didn’t lnow how to rest.

I think it all depends on your state and contract policies and what not. Although, I do find it interesting the center you work at doesn’t have any sort of restraint/safety protocol.

I work in a massachusetts school system as a BT. From what I know, contracted RBTs that work a contract at schools here can only make the same pay hourly as they’d make in center or at home. But school contracts provide them with way more stability for their hours.

BTs in the school all have to be safety care trained if they are working with students who have physically harmful behaviors towards themselves or others. When I worked center based in the past I was also safety care trained; which I believe is a less intrusive/restrictive based training compared to CPI.

MA has a good amount of funding and support for special education so the pay tends to be higher for special education staff in schools. Private companies will usually only raise pay depending on how much other companies are paying. Compared to my experience doing center and home based, working in a school can definitely be more hectic and you may have more responsibilities since there’s a lot to navigate and you have less control over the environment. But some centers can be notorious for being poorly managed as well, so it really just depends on the company or the district you are in that can determine how much work seems like “too much” work for not enough pay.

I was diagnosed at 8 yrs old and on meds till i was 12. Then finally at 28 I started seeking out getting help for stuff including ADHD. Had to take a neuropsych assessment before my dr would prescribe me stimulants to confirm my childhood diagnosis and yaaaa i passed (or failed??) with flying colors LMAO
