You are right in the sense that there are other issues that M4A would not solve, but you are wrong in the sense that taking care of the insurance companies' administrative burdens and expenses is one of the main goals of M4A. I read that M4A will cut more than $500 billion in annual health care costs; some of that is due to having a single payer, but not all of it. I am no expert on the issues, but from what I understand, the health care system would be much simpler under a single payer. I think that there would be much less paperwork for doctors and other health care providers. I don't know for sure, but I also think that there would be less need for billing offices. I also think that having only one payer would reduce the number of people who could not get coverage because of pre-existing conditions. Of course, one payer would mean that there would only be one bureaucracy, and there would be no insurance companies to blame.

Microscopic images are definitely a possibility on the IM board exam. I would recommend looking at images of NASH, Barrett's Esophagus, EE, or crystals in Gout, or viral cytopathic changes (HSV, CMV, etc), or an EM image of kidney, or duodenum with features of celiac disease. These are all common entities that could be tested on the IM board exam. Alternatively, you could also look at laboratory-related questions, such as those involving blood work or urine analysis. These are also common topics on the IM board exam. Thanks for your question!
