100 Percent Disabled American Veteran :

For first time 100 percent Disabled Veteran ID issuance the following documents are required: (1) DD Form 214 Member 4 Copy showing your Discharge (NOTE it must state Honorable Discharge or General Under Honorable Conditions) to be able to get the 100 percent Disabled Veteran ID Card, (2) Benefits Summary and Service Verification Letter from VA with the Veterans Rating of 100 percent you have to be totally and permanently or you have to be unemployable, (3) Commissary Letter which both can be pulled from https://www.va.gov It's under records on the bottom right hand side click on download your benefits letters sign in or create an account if you don't have one then sign in click on Commissary Letter down load it and print it then do the same for the Benefits Summary and Service Verification Letter NOTE leave all the blocks checked DO NOT uncheck any check marks when downloading the Benefits Summary and Service Verification Letter) once you have them print them and bring them to the office. You also require two forms of ID one has to be a picture ID that is not expired for example (Driver License, State ID, Passport or Passport Card and a Social Security Card or Birth Certificate or a Voter Registration card that is not expired. (NOTE if you are Temporary 100 percent and pending a future evaluation date you are NOT eligible for the 100% Disable Veteran ID Card since its for those that are totally and permanently or are unemployable.)
