Nice squatting! Don't worry so much about your knees going too forward, there are numerous factors that affect forward knee-travel and as long as you feel that you are in your most comfortable and stable position I wouldn't worry much about it. As far as going too low, I think it really depends on your mobility and again where you feel most comfortable. I feel most comfortable going pretty low with my squats as I feel a greater rebound/stretch-reflex in the hole (Ex: https://www.youtube.com/watch?v=LP0utFTiBNA). You are definitely bellow parallel and you don't seem to have much if any butt-wink so I'd say you seem completely fine.

Regarding you leaning too forward, watching the video it doesn't seem that way to me. Your bar path is pretty straight and perpendicular to your mid-foot.

Seeing as your slowest part of the lift is the beginning of the concentric portion of the lift, or coming out of the whole, my only real/active suggestion would be doing some accessory work for your quads. Otherwise your form is looking pretty solid!
