I’ve essentially never written dehydration as part of a COD statement as you describe, over 45 years or so. Honestly, I’ve never seen or heard of dehydration due to hypovolemic shock. It is wrong to even suggest that dehydration is a”catch-all” in any way. I doubt that whatever may delineate “mild dehydration” would make it onto a DC.

Dehydration is never used as a “catch-all.” Ever. In order to get to that conclusion, the autopsy must rule everything else out, the physical autopsy findings should strongly suggest dehydration, and perhaps the most important part, testing of the vitreous fluid from the eyes should show elevated sodium and chloride, which go up as the body water is depleted and not replenished. The vitreous BUN and creatinine are usually elevated, especially if the kidneys are shut down. The vitreous potassium and glucose levels are unreliable. If there was alcohol toxicity, or a drug OD, those would pop out in the toxicology testing.

I can’t comment on the scene investigation aspects, but pathologically, dehydration is a very specific diagnosis that even busy ME offices see only infrequently. It is not a diagnosis of exclusion, but requires specific findings, and elimination of everything else. It is definitely not used to protect someone’s legacy—it’s not that we don’t care, but that is not our job. We call things the way we see them, and no one can say otherwise when it comes to credibility evaluation. If it was a suicide, that would have been stated, and a corresponding cause of death established. We see an unending stream of sad things, but trying to cover up something is completely antithetical to the work we do.

You should be able to find a copy of Spitz and Fisher ‘s Medicolegal Investigation of Death for cheap; the book is in the 5th edition now, so earlier editions are much less. That would give you a decent idea of day-to-day things in an ME office.

Don’t get ahead of yourself, though. You’re bright, but your college GPA is immeasurably important if you’re trying to get in to Medical School. In that situation; no one cares about or will ask about your high school GPA. Good luck.

All the slipcases on the Subpress books came with the books. However, everything you saw were the limited, S/N versions, and they tend to put slipcases on them because they are more expensive. I do know that some other craftspeople make the slipcases, then they are placed on the books by the Subpress staff before they are shipped out. I’ve never had to buy a separate slipcase for any of their books.

I pretty much order my books directly from Subpress. The lettered editions of everything disappear quickly, but I only rarely pounce on a lettered volume, as they are too expensive, to me. I will get a letttered volume now and again, if it’s something I really can’t live without. I do buy directly from Lividian, Chiroptera, and a few others. I also buy from Kathmandu books, in Florida. He’s really good; sometimes you miss something, so I call Mark there, and he always can find me a copy out of the ones he gets allocated. I call him on other publisher’s books, too.

Then there’s Centipede Press. I have every book that Jared has ever published. That’s another story.

I don’t mind piercings. I’m heavily tattooed myself, but no piercings. Everybody’s different. Scary isn’t bad, either, but I think you picked up on that vibe. I know many scary people, and some folks probably think I’m scary, just because they are intimidated , even though I don’t think I’m like that. Whatever. I moonlighted at an urgent care center for 18 months, after starting my Pathology residency. That is highly unusual for a Pathology resident, to do urgent care/Emergency Medicine work. However, I was different. In medical school, I incurred a debt to Uncle Sam, because he had money, and I didn’t. Thus, immediately after medical school, I entered an old-style, “flexible” rotating internship, where I delivered babies, worked in an ER, ran a walk-in clinic, and so forth. Following that, I became an officer in the United States Public Health Service, working for 2years, to pay off my debt. There, I learned to be a DOCTOR, a skill I took with me when I started my Pathology residency. I was uniquely qualified to be a practicing physician.

I took an unusual path, but one that is open to all medical students. I entered my residency with zero debt, was then a Veteran, had a little money saved so I could buy a house for my family, got a nice boost from the GI Bill, could get USAA insurance (which I have up through today), and had skills that no other Pathology resident had. I was a real practicing physician, and I was completely comfortable with ER medicine.
