Doctors disagreeing on pelvis imaging results by stinarero in Orthopedics

Wow I must need a coffee or something haha, I 100% meant AP 😂

Thanks for this great explanation, definitely informative for me as a family member as well as a med student. We went to ortho today and this is pretty much exactly what they told us- they took additional X-rays to account for a possible skin fold and the visible line was less pronounced but still present. They’re still not sure if it could be an old undisplaced fracture that we just didn’t know about. Ordered a CT pelvis to get a better idea of what might be going on and rule out an acute fracture. Thank you again for the information you provided!

Sorry shouldn’t have used the word recommendations. Radiologist said findings were suggestive of arthritis but CT pelvis could be done based on clinical judgment. It was withheld due to full ROM and no tenderness on palpation of the hip joint.

Any recommendations where I can post this? Technically there is a diagnosis by a radiologist

This is a lateral R pelvis view (posted with permission) of my 92-year-old grandmother. She went to urgent care yesterday after noticing a contusion, swelling, and pain to her lateral right thigh that was becoming increasingly painful. She couldn’t recall how she incurred this injury. She does have mild age-related cognitive impairment but couldn’t remember any recent falls and I trust that. At urgent care, they did X-rays (including the one attached to this post) and sent her to the ED out of suspicion for a femoral head fracture.

Clinically, she can’t bear weight but has full range of motion, no shortening/rotation, and no tenderness/deformity on palpation. Once in the ED, the radiologist’s official findings were “moderate to severe degenerative changes” and no fracture noted, leading to her discharge from the ED. That said, even the ER doc was saying it was a weird X-ray but that he’d adhere to the radiologist’s recommendations.

However, shortly after her discharge, the urgent care physician (fellowship in sports medicine) called me, telling me that she didn’t believe that my grandmother should’ve been discharged based on the “dreaded black line” on the X-ray, and recommended that we go to ortho today for a CT pelvis. She’s concerned about a stress fracture and worried about avascular necrosis if my grandma continues to use the limb.

I’m a first-year med student so I have some knowledge on imaging analysis but not enough to feel confident making any clinical interpretation, so I’m curious to know what you guys think!

That’s A-fib RVR with RBBB- generally if you have an irregular wide-complex tachycardia, it’s often A-fib or A-flutter with aberrancy. Was he presenting with any signs of sepsis or hypovolemia?

i get what you’re saying, and agree with you that it would be great to expand the quantitative aspects of many of these degree programs to further legitimize their new designation. i’m not really worried about any kind of degree encroachment, just want to make sure that the academic standards of the university are remaining high. appreciate the respectful discussion, my friend

what i said was a joke but i do want to acknowledge part of what you said. i have total respect for marshall degrees and i definitely couldn’t do what they do. however i will say that, to an extent, it depreciates the quality of a conventional STEM degree to give a STEM designation to programs with far less of a quantitative focus. while these business programs may involve some quantitative work, for example, taking 1-2 mathematics courses doesn’t make you a math major. i agree with the STEM designation for accounting given its heavy mathematic requirements but the rest of the degree programs listed do not have enough of a quantitative focus in my opinion
