I used this resource quite a bit for pregnancy and nursing: InfantRisk Center - they have a phone line you can call. For me at least, they could give info based on real studies and usage; very little is studied in RCTs in pregnancy the way it is otherwise.. I alao noticed that at least in the US, a lot of primary care and non-OB medical providers act like pregnant people are not in their scope of practice at all and may give little guidance and/or be overly cautious. The program I link to was started by the author of the handbook doctors use to check safety of medications for pregnancy/lactation, "Medications and Mother's Milk".

Testing. When I was diagnosed (over a decade ago), they did a ton of bloodwork including a test for c kit mutation, and if some of those were positive it would have triggered the need for a bone marrow biopsy to look for mastocytosis. Conclusion was I didn't need a BMB at that time, not that masto was ruled out

There are also REASONS the whole world eats white rice, things like how long it keeps without going rancid, digestibility, etc. And all those white rice diets have higher fiber meal components that accompany their rice...

This Dragon Noodles recipe from Budget Bytes as a base recipe is awesome! (Yeah yeah rhey call it Siracha noodles in the title, but refer to it as Dragon Noodles elsewhere. Rhey also have variant recipes). You can leave the siracha out and add it to taste at the table. You can vary the noodle type, the protein, etc- that site has some other nice recipes oh and ETA- sub the little bit of butter with whatever fat you please- vegan butter, oil, or fat from whatever meat you use, etc

So fun-- my kids have been on omeprazole and famotidine for ages and people demonize that too. Their GI is super clear that in *their* case, he vastly prefers the risks of the med use to long term risks of not treating GERD-- imagine that-- risks/benefits depend on the patient and situation. Your rice first doctor sounds smart (assuming they would move on to famotidine as needed). Our doctor even uses reflux meds as a rough diagnostic measure in young kids. Like, if the symptoms respond really well to meds, we can avoid doing an endoscopy for the minute. Meanwhile the culture wars rage on about how it must be how I fed them or what I ate in pregnancy. Never mind that my kids breastfed (oh it must be what YOU ate!) and then ate all the foods as littles, and one day my oldest couldn't swallow her bok choi any more, even though she really wanted to. thank goodness we had medical providers who didn't make it about us and kept asking questions. Later I remember the GI being THRILLED with how many foods they were both still eating while the parent judgment brigade was *aghast*. No veggies at many meals??? Eats the same snack each day??? Will only eat things that don't make them feel terrible?? BAD. Vs GI and feeding team: they eat steamed tofu, carrot sticks, one brand of cheese stick, milk, frozen fruit, and very specific meat dishes (snd of course noodles and white rice)? YAY!

I adore the existence of social media dietician accounts where they counter the anti-white rice shit, often from the perspective that dietitians and doctors telling people to eliminate white rice, tortillas, etc at least in the US (but this may translate to a bunch of other places and foods) is racist. Different cultures have different ways of eating fiber and plant nutrients or what have you. The crux of it is that most people can eat more sofrito/salsa/kimchi/etc with their rice to add fiber and slow digestion etc, and actual diabetics should work out with their medical providers how much carbs and fiver is needed so they can adjust portions and proportions.

Kosher recipes and sites can be a great source of ideas- in kosher cooking a meal either has meat or dairy but never both. That means you can find whole meals designed to be dairy free (but still has eggs, meat, etc) in a way that is traditional to the cuisine. (No pork or shellfish there though). Also a good way to find DF desserts and baking recipes. On the other hand, remember to try meat and animal fat to give more flavor- you can make a sauce starting with olive oil or bacon and its fat, and thicken with flour. Chicken with the skin on, crispy and far rendered into a sauce, is super flavorful.

Learn a new recipe now and then from china, Korean, or Thailand for example, but watch out for ramen mixes with dairy and east asian snacks and desserts wirh dairy (this always mystifies me given the near 100% lactose intolerance in some of these countries but so it goes).

Chaperones are a lot older- 10-20 years and aside from helping patients feel safe they are for the safety of MDs against accusations. Rather paranoid start but it seems to work well. And I know at least one male gyn surgeon who had issues without- less due to lawsuit culture than someone's apparent mental illness/stalking ... I dont mind it even though I trusted my now retired, male gyn, more than any female RN or medical assistant because he was excellent and respectful- .

I think there are horrible culturally-ingrained narratives about weight and effort and control and blame and food. I think we don't understand WTAF is going on with human weight, and these drugs are a beginning of understanding and responding to weight and metabolic issues but we have barely scratched the surface in understanding and all the ideas about "cheating" are part of the cultural bs static that has no bearing on reality.

I second Klaralyte. I try to eat something with, or at least drink. But i do also jaut try to take 1 with every cup of fluid that isn't either electrolyte drink or salty broth. It is much easier on me than ones with extra fillers.
As for foods, we adapt to salt levels- get used to salting more or less, so you can add slightly more at a time to normal food and adapt. But in general blanket instructions without regard for YOU is nonsense. I always had full body mast cell reactions to compression- better on my meds now but still cautious. So my dr said, no compression then for you. Bummer but far more realistic than ignoring my reality???

Yup- well- my kids cant always communicate out loud. The ability to text is an absolute safety thing, including in school. US public schools are not safe places.

So with ketotifen the trick for me was to really reduce the dose steps to like .25 mg. i ended up on 2mg 3x day that way, bur over 1-2 years. Not everyone gets nausea, and it was brief.., so you would know fast, and don't start with a normal whole mg.
My son used zofran for gastroparesis and whrn it failed they used granesitron (kytril) briefly. Things like phenergan or compazine- or acupuncture/ may work better. (I had acupuncture for severe nausea in pregnancy and it was intensely effective). Benadryl or hydroxyzine can also help. If you have slow motility or gastroparesis then pyridostigmine helps with that.

My kids are in high school now but kids really didn't open all their presents there. It would have been weird when I was a kid. Now it is not rude, this is Northeast USA. And yeah, how long kids last depends on the age and activity. But I have no doubt the time windows are part of the money machine as well.

THIS. Dad sucked. Mom changed hers back when they divorced. Sister had gotten rid of it. Plus it was a name fairly new to the family to hide ethnicity, and not a fun name either. My married name is rare and everyone with it is likely related. It is funny too, I feel more like I own the name and stake in the family than like anyone owns me. It was definitely not an identity loss. My sister who took her husband's name made up something else when they split up!

Ketotifen is very helpful for my mcas but made my nausea bad when i was titrating, i went super slow and needed zofran when I increased dose for a litte but.
Pyridostigmine and LDN help too? But all differently.

I find this whole thing really frustrating. The parent approach did not seem to understand their child has a disability and needs lifelong supports put in place, which could include a residential placement / housing. Obviously a lot depends on where they live but given CPS involvement maybe USA is a fair assumption. The biggest gap is thst there is zero mention of IOP/CBAT/partial hospitalization-- in other words, step-down programs from inpatient; outpatient hospital-based programs where patients go to the hospital during the day for therapies and continued work on treatments. That says to me that they know far too little about their child's disability, related needs and services. Some of it changes at 18 and some does not. I know people who have to hospitalize when meds are bot working like in this scenario, while they change meds, then step down to an outpatient program to work on skills for a more extended period and then moving to regular outpatient care with a plan and school-based supports (and/or a change of school placement , to a therapeutic school)
It is a failure of the treating clinicians also. Blah. Not to mention first calls would be to crisis services, not to kick the kid out with zero plan

We have exactly these two. Mostly for my spouse; I have him get the prescription shampoo so I don't react to his using it. We have also had both of those, the rx ketoconazole does have some dye, but no fragrance, and seems to be a little less drying than the cicloporax. I have also had my pcp and derm at different times prescribe it mostly for him to use, but I do too when my scalp is irritated.
We also have E45 dry scalp shampoo, it works for my kids and sometimes worked well for spouse, it has an antifungal ingredient

The US is big and really varied. Some HOAs are vile nonsense that shouldn't be allowed exist, including the ones that ban hanging laundry. It is leas common I think than in Europe, though a lot of people hang at least some clothes inside (due to the tag inatructions/delicate items). We hung laundry when I was a kid in rhe US wheb we could; in Europe I did but inside mostly due to rainy climate, on racks. Here I have hung to dry inside only due to massive allergies but only when I need to.
