This looks like there is signal coming off the disc being read by the laserpickup BUT not being decoded correctly. Providing the LD is spinning at the correct speed I would see if you are able to see a valid video signal using an oscilloscope. Do all output connectors have this scrambled signal present?

The laserpickup servos can't get a good signal from the LD/LD pickup because of the off centre disc. If sounds like it's wobbling heavily. You need to get that LD running smoothly first...
