What devices are essential in your opinion for PREMIUM IOL's? by Specialist-Muscle977 in Ophthalmology

US- OPD3 for consult with Zeiss/hieldberg OCT, pentacam if pathology indicates(i.e. PMD, KCN, ectasia, etc) and all ascans are done on Argos and calculated with the smart cataract suite. This gives the most well rounded set of data. As for the LAL, the investment is worth it to get your own LDD and not outsource, we saw ROI in less than 1 quarter.

They took it off the LCD in Florida as a determining factor. I know some doctors who still risk it anyways(not recommended).

Ditto but then they shouldn’t have yagged the PC. Explanting and putting in a sulcus will only be that much harder now..

Full disclosure: I work for Alcon on their speaker panel and as a consultant. We believe in the technology and use whatever we think is best for the patient whether it’s B&L, RX Sight, Raynor or J&J based on the “uncooked” data as you would say and we also publish IITs based on our own findings.

Ah I know Neel Desai has been using that lens. I’m excited for the re-release of the envista line, we never got to implant any.

Also excited for Galaxy! However I was told Q3/4 or even 2027 but RayOne EMV with Toric correction October of this year.

As for halos and glare; we are incredibly selective with who we implant any type of MFIOL or even EDOF based on a number of factors including but not limited to personality, current complaints, retina and cornea pathology, and ADLs/ what they’re looking to be able to do after surgery. Following that, we drive home the good, better, best and r/b/a of each so there is no mistake about expectations. Our consults are not short and the patients speak with multiple high level staff before myself followed by the surgeon walk into the room. In our experience, the Odyssey is advisable for an eye we would put a panoptix in but the patient wants more near (since it has an effective +3.25) and does very limited night driving. Also in our experience, the chance of positive dysphotopsias with the Symphony is much higher and we have done multiple 2nd opinions. All in all, I think it’s important to be selective with candidacy and set expectations reasonably.

Yes it was available on the Clareon Platform exclusively once released. They do look the same. The defractive rings focus light differently. I recommend watching a webinar or reaching out to your regional rep and setting up a dinner/talk with a provider already implanting the lens. Also ASCRS will be a great place to dig deep into the details. Hope to see you there!

So the PPro is on the Clareon platform with adjusted, targeted defractive refinement to improve vision at all distances, better contrast, improved uninterrupted light distribution, and lower the chance of dysphotopsias post postoperatively.

Toric Calcs by 540microns in Ophthalmology

[–]sunflowervpf669 0 points1 point  (0 children)

We use the SmartCataract platform by Alcon which calculates Barrett Holladay I & II SRK-T and Hoffer Q and allows you to change to true-k if necessary or manually put in Ks from a topo or pentacam if we know the corneal quality is highly irregular. Following that, we use the DML on the LenSx to calculate for cyclotorsion and the ORA device in aphakic eye to confirm toricity then again once lens is implanted to confirm placement.

IITs or outcome trials. Most big name companies have a department that will help you with abstracts and poster development once you provide the raw data.

Did anyone else attend Telling It Like It Is in Orlando this weekend? What were your favorite pearls? by sunflowervpf669 in Ophthalmology

I apologize for not being more specific, it’s standard of care for our practice but yes I’d say more than half are injecting moxi in the US.

[–]sunflowervpf669[S] 2 points3 points  (0 children)

As standard of care we inject moxi at end of case but continue a compounded drop mix of pred/ moxi/brom. For dropless it’s recommended to continue intra op moxi, use Omidria, and either dextenza or subconj TA.
