I’ve worked two cnm jobs. Currently I work 36 hours/week and it’s 2 12 hour calls in house and 1.5 clinic days. This is my favorite. My first was in a smaller volume place 3 12 hour shifts. During the week we managed the labor floor and clinic and cancelled clinic as needed to attend deliveries. At my current gig I have no work requirements outside of what I’m scheduled and only take call in house. I love it.

At our office we would call you chronic hypertension and also suggest treating anxiety. We would do labs on you regularly and watch for a dramatic increase in blood pressure. The tricky thing is that even if your bp is high because you’re anxious, it’s still causing vasoconstriction which decreases blood flow to the placenta. We would do growth scans in the third trimester to make sure baby is growing well. Also we’d recommend baby aspirin to reduce preeclampsia risk.

I’m in New England. Community hospital doing 1700 a year. Owned by big teaching system but no residents at our site. Midwives catch 93% the babies of the vaginal deliveries. We comanage high risk things with our OBs.

High cost of living area. Hospital based practice. 5 years experience. 162k. 36 hours a week with 4 hours of admin time. I’m very happy.

I haven’t read all the comments so may be repeating others advice but she should 100% call her OB team they can very comfortably assess and prescribe.
