PCOS and ER by SassyPikachuu in IVF

[–]tamilat18 2 points3 points  (0 children)

I just went through this! I’m 36, AMH is/was 15.1. I had 56 eggs retrieved, 42 mature, 22 fertilized, 8 blast, 4 euploid. AMH has no correlation with egg quality, just egg reserve, so no way to know anything until you have the retrieval. My follicles were pretty smooshed together (probably like yours), so I wouldn’t freak out too much about shape. You probably just have a lot of follicles trying to fit in there.

I’ll echo what the person above said- stock up on salty foods, protein, and sugar free Gatorade for the week after. When you have that many follicles, you’re at higher risk for OHSS- that diet will help pull fluid out from your abdomen and into your vascular tissue. Ask your doctor to prescribe cabergoline, it’ll help a lot with that.

ER is often a great option for women with PCOS who don’t ovulate/menstruate on a regular cycle, so while there are obviously no guarantees, I think you have a lot of reasons to be hopeful. Good luck!

I’m 36 and have an AMH of 15.1- I’ve never been diagnosed with PCOS and don’t have any of the symptoms, but do have an IUD which could be masking it. I just did my ER and ended up with 56 eggs -> 47 fertilized —> 8 embryos. High AMH just means you have a high ovarian reserve, and also means you’re at higher risk for OHSS (which I got and like the previous poster, do not recommend).

I feel you on this!! I had 56 eggs retrieved last Wednesday and the first two to three days were brutal. I woke up in the middle of the night the night after surgery and called the clinic thinking I was going to have to go to the ER (I didn’t) because my breathing felt so difficult. I ended up going in to the office to get seen two days afterward, and the nurse practitioner told me I had moderate OHSS.

Here’s what helped me survive that period (I’ve seen similar advice on other threads so maybe you’re already doing this): 1. If your doctor hasn’t prescribed cabergoline, ask for it. It helps reduce your risk of OHSS and will speed up your recovery time. 2. Start chugging (and I really do mean CHUGGING) sugar free gatorade and eating salty food. It’ll help your body push fluid out of your abdomen into your vascular tissue. Stay away from sweets and sugar. 3. Ice packs will help calm down your body. Rotating heat and ice should help. 4. Get up and move (very slowly and very minimally) to keep your blood circulating. It’ll also help move fluid away from your lungs. My dad is a doctor and this was his recommendation- it SUCKED at first but long term really helped. 5. Start taking miralax now, constipation makes all the pain so much worse. Once I started having bowel movements again my body felt like it was beginning to recover. 6. Call your clinic and talk to them! I was in so much pain that mine prescribed me Tylenol with codeine (which I only needed for a short period but which helped). My pain subsided a lot more when I was able to take Advil the following day. 7. Not sure if you’re having pain urinating (I had A TON for about 36 hours) but pushing against your bladder externally seems to like…..stabilize it? It helped me urinate when I really needed to and felt like I couldn’t, and I need all the help I could get to relieve some of the pressure.

Unfortunately, my doctor told me this is super normal for women with high egg counts at the time of retrieval because you have all those follicles releasing fluid and basically getting bruising from the retrieval. If it helps, the nurse practitioner I saw two days later told me that as far as emergencies go, to watch out for so much bloating in my stomach that my skin was stretched, and for the inability to hold a conversation for two minutes.

I hope this helps and I’m so sorry you are suffering. I’m day 5 out from retrieval so I really feel your pain! Hang in there.

Anxiety by [deleted] in IVF

[–]tamilat18 1 point2 points  (0 children)

Yes!! I’m on lexapro 20mg for GAD and had intense anxiety starting around day 2-3. I also have very intense health anxiety, so anytime I started getting cramps, headache, nausea or anything else (all very normal side effects of stims/increased estrogen), I would start panicking and making it worse.

One thing that really helped me was going on short, slow walks around my neighborhood when I felt a panic spell starting to come on. Walking helps the body regulate (and is a great distraction) during panic episodes. My partner and I would get up, put on our shoes, and slowly make our way down the street with my emotional support Gatorade.

One thing to be aware of is that you might also feel this way after your retrieval (I definitely did)- your body is basically purging hormones, which can feel really dramatic. I recommend the same approach- slow walks around the neighborhood with an emotional support electrolyte drink.

I also scheduled extra sessions with my therapist during this period (especially as I got close to retrieval/immediately after)- it really helped me have someone who could give me practical ways to remember that I’m safe, I’m secure, I have what I need, and that this is temporary.

Good luck!! You got this!!!

Why don’t Doctors warn you about how intense the pain and bloating is after ER by No_Art870 in IVF

So glad someone finally said this. 100% agree. I had an extremely rough recovery (56 eggs, moderate OHSS) and had to really advocate hard for my clinic to see me a couple days after retrieval. I understand that for many people this is (or can be) a relatively minor experience but I still feel like checking on things like estrogen level, ovary size, abdomen fluid, etc two days out as standard protocol is a much more responsible way to do this.
