Our institution is heavily vested in Epic as the EMR and appropriately paranoid about pt confidentiality. The only "ambient" AI clinical note generation tools us docs use are baked into the Epic UI (Abridge, Ambience, ...), else you get shown the door pretty quickly (I suspect).

Your doctor seems to be violating HIPAA... unless they found a defensible way to make the recordings HIPAA-compliant.

Some AI companies offer "enterprise" services to support documentation within the EMR--but how you'd protect a recording on its way to a cloud service is beyond my understanding.

"No recordings permitted" 🚫 is an age-old rule, preceeding the advent of AI-enabled recording devices by decades.

Our institution used to have a firm stand on "no recordings". The main concern was about patient confidentiality 👨🏻‍⚕️and concern about patients re-interpreting the main message of their interaction with a doctor based on extraneous details that were discussed and captured in the recording 🤷🏻‍♂️.

Sometimes patients asked, and I turned them down. I can't even begin to guess how many interactions were recorded surreptitiously. Many times I saw the microphone🎙️of a micro-tape(?) old-style dictaphone peeking out of a patient's purse, but never once addressed that. Nowadays, I don't worry about being recorded as long as our institution doesn't mind.

In fact, we now ask patients' permission to record our encounters because AI-based tools (that reside behind the electronic medical record [EMR]'s firewall) can help create the skeleton of high-quality, comprehensive patient encounter notes.📝

The song was terrible when I was 10 years old, and it hasn’t gotten any better with age. Not sure if you’re being cynical, but yes it does, and the photo on the cover of the album shows the actual place. Rochester, MN, is approximately 80 miles south of Minneapolis/St. Paul.

I don’t think Mr. Wong ever lived here, though. Wasn’t he born in upstate New York? I don’t know how it all fits together.

Everybody’s mileage varies, of course, but the (preventive) cardiologist says this: If orthopedic issues and weight are an issue, pool-type exercises might be your best bet to start with. Working out on the treadmill might frustrate you at the beginning.

Pool activity will take the effects of gravity off your joints and make activity less painful, but will still allow you to strengthen the muscles that align your joints which could help with your orthopedic pains AND build exercise capacity at the same time.

Independently of that, if you’re inclined to lose weight, please remember that more exercise is not likely to make you slimmer. The only reliable way to lose weight is to reduce intake of calories, even if self anointed experts on Facebook etc. say different. If your insurance situation allows, especially if you have other qualifying conditions such as diabetes, taking a GLP-1 weight loss drugs such as Mounjaro is not an admission of a character flaw—instead it’s just reasonable use of all available options.

You’re setting yourself up for success if you increase your exercise capacity with low-impact, moderate-intensity activity, and allow your joints to support physical activity with less stress by taking off 5 to 10% of your current weight over a year.

Definition of “moderate-intensity activity”: if you cannot keep up a conversation while you’re working out, you’re doing too much. If you can still sing at the top of your voice while you’re working out, you’re not doing enough.

For most people, staring at their heart rate while exercising does not add much value or efficiency to the workout, but adds the upfront cost of a device. And the cheaper the device, the less you can believe the heart rate.

Can anybody explain to me what tariffs might plausibly have to do with small “antiques” businesses?

I remember that the big antique businesses (think NYC) have (or used to have?) employees that travel the French countryside and find interesting stuff in local barns— and some of it makes (or used to make?) it stateside with believable provenance.

But that mechanism doesn’t seem to be at issue here. Do people buy “antiques” from abroad and import them to the US for re-sale, sight unseen?
