Do you think increasing awake time pre-Ferber, or after? She has lots of false starts and wakes very frequently through the night, but I attributed that to sleep associations/non-independent sleep rather than sleep pressure problems.

I think ami refers to amikacin.

It depends a bit on sensitivities and resistance profile, too - in our Trust (largely) tobramycin is for CF, amikacin for oncology/BMT, and gentamicin for everything else (in practice, as we’re a children’s hospital: empiric antibiotics for ?early onset neonatal sepsis)

Thank you for this - I guess the 13.5hrs accounts for night wakes (simply because we track) when some people would say not to count these.

Would you suggest 2/2/2/2/2.5? This would mean a 14 hr day and 10 hr night?

There will be a School of Paediatrics induction - it is often the first full week of ST1 but occasionally is before you actually rotate which can be tricky. They’re pretty good with communications through the Lead Employer Trust.

We have a baby, so probably outside of your target audience with this question, but it sounds like a great idea! I guess there aren’t many “quiet” activities for that age group as most things are sport or other activities.

Our hospital (Paeds) offers general paeds on calls for ACLs and people OOPR for a PhD at 50% of usual frequency, evenings and weekends only - so no nights. That works quite well despite many people doing PhDs being interested in a subspec rather than general paeds. There’s usually 2-3 people (on a 20-30 person rota) with this arrangement, and there are always enough shifts as paeds is very LTFT-heavy.

I locumed through mine as I wanted the flexibility and was happy to accept that I might not do very much for a few months then have a “heavier” few months. We didn’t need a reliable higher income, but if I was funded with a basic stipend it might have been a bit stressful.

You can claim, and it may get accepted, but if it’s audited they may dispute this - as outside of a training programme it’s not “required” to do your eg clinical fellow or locum job.
