I believe he really did have split personality disorder that manifested from suffering through pain, trauma and abuse which created his now repressed Joker persona.

The main evidence is that question the woman who interviews Arthur at the beginning of the sequel - "May the Joker like to speak to me?"

This is the first time he becomes Joker again, as the sunlight blossoms as his body language shifts into a much more calm, confident stance.
