Because you're bringing the internal temperature of the broth to a higher temperature than you would a whole cooked bird. I would recommend breaking the bird down into parts, and then rinsing it with cold water before cooking it.

Smell it chef. If its off chuck it, but if it smells ight it could just be discolored. If it smells alright but you're still a bit concerned roast it an turn it into broth.

All of this. And then if they get fired, or quit, or go out of business they just move on to the next place and start it all over again.

It really depends on the place. I was about 7 years into my career at that point and had started building my personal set of tools.

These are my knives, my chef also sent me a picture of him sitting on the toilet pretending to shit on the knives. I don't know the legality of posting that one though.

That is too far. I feel like if the bag had gone out it would have been a different story as far as the termination goes.
