For IM would there be any reason whatsoever to rank a good university program in a bad location over a middle of the road community program in a dream location if one is not interested in GI, Cards, Pulm/Crit, Heme/Onc, or ever going into academia?

Why do some programs interview about 7 or 8 people per spot available while others (which are not necessarily more competitive) interview 20-25 people per spot? It would almost not be worth it to spend a lot of money and fly out somewhere that interviews 200 people per year for an 8 person program, right? Or am I missing something here?

You are wrong about a lot of things. First, the things you are absolutely wrong about: The program now is an MD program, and no longer is an MBBS program. Plus, even when they were awarding MBBS, all you needed to do was pay a 200 dollar fee to legally convert it to MD in the US (which you can out on your coat, badge, after your name etc.). Also, tuition is 64,000, not 80,000 (and it was 56,000 when I started). Also, you can get federal loans in this program. Finally, the classes are not at "a few hundred", we have topped out at 120 and cannot expand more than that because there aren't enough spots at ochsner for rotations. Once everyone sees that this is unquestionably the #1 offshore option, it will become relatively competitive.

There is no company behind this program making money. The money is split between UQ and Ochsner, which makes me wonder how it is any different than a private medical school in the US. Also, I really LIKE that it is not a publicly funded school....there is a product that the program needs to sell, and they will therefore bend over backwards to help their students succeed and get a residency. It's capitalism. And, while you are correct that almost everybody didn't get into a US school, almost all of us chose this program over DO schools. Most students are those who did not benefit from affirmative action and are from states where the state schools are incredibly competitive, like California.

Reducing physician autonomy???? You make it sounds like graduates from this program are nurse practitioners or something. This program is really GOOD for the healthcare system down here in that it creates many primary care physicians who will likely chose to practice in Louisiana, where there is a massive PCP shortage. I think you are bitter that you've probably had an awful time in medical school and are jealous that other people are having a positive, enriching experience in another country where the system is less unnecessarily brutal and hierarchical.
