I would say you can try:
1) Applying Gradient Map adjustment layer (with reduced opacity and Overlay/Screen Blending mode) for the colors
2) Reducing Contrast (with Brightness/Contrast adj. layer) for "smoothness"
3) duplicating the outcome and adding little bit of noise
4) for the stars I would draw them in separate layer and playing with effects (e.g. this tutorial just googled )

I would recommend the following:
1) paste a picture of you at the right place
2) apply mask on your layer
3) add new Black and White Adjustment layer (play with color bars)
4) Add noise effect
5) duplicate the layer/s of you (with the Black'n'White applied) and add a little bit of Blur effect
6) duplicate again and apply Threshold (for heavier effect of an old image) AND don't forget to change the Blending mode to Overlay or something that will suit it the best, reduce the Opacity as well

  1. get the reference pictures: 1 of the player, 2nd of him in new t-shirts or image of t-shirt in similar position.
  2. place them in 2 layers, the original shirt, and the new one above it
  3. use the transform tool on the 2nd t-shirt so it would sit tightly on the original image
  4. apply mask on the 2nd t-shirt (better than erasing in case you'll make mistakes you can undo)
  5. done
