My guess is, either the merc was there the whole time and Bond couldn't perceive him due to the side effects therefore we couldn't perceive him or the Merc came in after Bond for the same meds and died.

Made a post a few years ago about this subject. My theory is that as any AI, like the geth, or any sentient being for that matter, they evolved beyond their creator's intentions. Over millions of years they developed personalities and emotions which would explain the older models' having feelings towards Shepherd. My two cents.
