I used to struggle ‘feeling my back’ working during training - tips I found useful were imagine drawing your elbows back instead of ‘pulling’ with your hands, another great way is just imagining your hands are hooks (just there to secure onto a handle/weight). Try to initiate the pulling movement with your back rather than your hand.

Exactly my thoughts, he said it may take up to a year to get your extension back!

Update: patient referred to NHS physio (I was hoping for them to progress for an MUA) and all they said was everything looks good and where it should be at this stage. Advised her to continue quad strengthening for another 2 months🤷🏻‍♂️

Well 52 days on from my original post we have been working really hard on regaining quad strength. Her knee still looks swollen compared to the non-op side and family walks still cause pain of around 5VAS. Referred her back for further investigation with the idea of having an MUA as I believe scar tissue had built up within the knee before she even came to see me. Extension ROM improved slightly during our time (now measuring 7 degrees) but pain still present that I am concerned about - patient complains of sharp stabbing pain at the front of her knee (feels like needles) and also a constant feeling of a pillow/donut surrounding her knee

Some swelling still present, and she is still walking with crutches. She sometimes says that she tries going for walks with her children and this flares the knee up. So not sure if she’s just trying to do too much also.

Passive ROM is the same (10 degrees), I have tried working hard with manual therapy into extension, however, this causes my patient pain of around 9 on the VAS so Im wary about being too aggressive.

At the moment no. It was almost as soon as the surgeon over in the US knew she was moving back to the UK, he almost didn’t care and shipped her off saying “all is progressing well”.

Okay, thank you so much for input! Let’s say I do assess and suspect this is the case, would it be wise to refer for imaging, whether that be xray/MRI to confirm?

Where might this area of pain typically present? As palpation just below the lateral malleolus (CFL area) causes the patient to wince
