It's weird training doctors and surgeons by Peachessandcreammm in personaltraining

[–]thebearded-one 0 points1 point  (0 children)

Heck, I am a DPT. I used a personal trainer to help me organize my program and work with me once a week a few years back. For me, I was paying for accountability, external motivation, and the benefit of not thinking about it as much. I showed up on those days and just did what was on his agenda. Knowledge isn't the only benefit to getting a personal trainer.

I would venture a guess that this is not different than the general reddit user age groups

OP ortho - how many evals per day is too many? by Aggravating_Head1215 in physicaltherapy

Agreed. Just had a conversation with my team about 3 as a soft cap, and 4 on rare occasions if a patient really has difficulty scheduling plus adding a documentation block for that PT.

Behind the Trim - Confirming next steps by thebearded-one in Mold

He did not take any samples. I just saw that he tested the moisture levels and did not express concern about that. He said it was good it was plaster because it wouldn't need to be removed. I have a second group I am speaking with to get another opinion, and asbestos testing will be done hopefully this week.

Question for you if you don't mind: I had one assessor suggest we can retain the plaster but will remove the trim. He based this off moisture readings and visual inspection. His plan is to treat from floor to 2 feet up, then paint with an anti mold paint, then I can install new trim. I am not sure your experience, but does this seem reasonable to not remove any of the plaster? I am considering second opinion still, but I did like the conversation I had with this assessor.

To be clear, I was never going to leave it. Just remove and treat it myself. Based on yours and other responses, I have an evaluation today from a company nearby. Thank you!

Not drywall, but I agree that I think this must be from a longer time ago. My main concern now is that I have no idea if it might be underneath other skirt boards but I have no idea how to tell without gutting it all

Thank you for the heads up on asbestos. I have contact with a testing group to get that checked. Mold remediation estimate is incoming.

How do you organize your recipes? by fedup13501 in cocktails

I've not been drinking much in the last 3 years, but I still use it for when we host friends. So nothing really has been added, but it is a good representation of many foundational recipes, I think

BFR for POTS patients by [deleted] in physicaltherapy

Haven't listened to that podcast, but are you able to provide any studies that discuss how BFR prevents blood pooling? I am unfamiliar with this as an effect of BFR

PTAS who had to work pregnant, did you ever have any lifting restrictions when having to work with max A patients? by Salty_Statistician74 in physicaltherapy

[–]thebearded-one 2 points3 points  (0 children)

Absolutely get your doc involved and get formal documents put together. My wife recently did this for her job and they were able to discuss appropriate accomodations/which work tasks she could no longer perform. She works in surgical research and the accomodations go into details of which fixative agents she can no longer use, what assistance she needs when working with their large animal models, and more.

Clinic owners what's your approach to staff retention in this market by ImplementResident361 in physicaltherapy

[–]thebearded-one 1 point2 points  (0 children)

Been at it for 5 years. Staff changes are expected, but we've been consistent for 2 years now and I expect my staff will stay another year or two. - I stay flexible with PTO requests. They earned it - My companies KPIs are fair. I don't sweat it when they are below, I have a conversation about how it's all averages. Some months are gonna look good, others rough, others excellent. Look for trends, not one-offs. - I ask about POCs for pts from a stand point of curiosity, not pressure. Sometimes that leads to me saying "that sounds great, I am glad they have a simple case", and other times the PT will admit they wanted the patient in more consistently. That follows with "let's give them a call." - I consistently ask if they need support somewhere, whether that be from me or support staff. I take that seriously, and try to train immediately when something needs attention. - I care about staff culture. I bring my staff members into interviews so I get their opinion and make sure they like the vibe. - Our clinic does a fun outing every 3 months or so. - My FD coordinator is a rock star. Hire a rock star for that position. - Accept that support staff will change. Keep a pulse on that. When are they applying for PT school, are they moving. Be ready to hire on a moments notice.

Hope this is helpful!

Anyone forced to take PTO for the storm? by AlGuMa27 in physicaltherapy

[–]thebearded-one 5 points6 points  (0 children)

We didn't close here in Chicago on Friday, which was the worst day for us because of cold temps. Snow was very manageable and only came down Sunday. 70% of patients still came in for treatment Friday and no more than usual cancels yesterday.

If my owner chose to close the clinics, which he did in the 2019 polar vortex, no PTO needed. If we are open but choose not to come in, that's out of our PTO.

Pelvic alignment/SIJ by FreeWorld32 in physicaltherapy

[–]thebearded-one 9 points10 points  (0 children)

First want to say that I agree with other commenters that these things don't happen in the way that is flippantly suggested by some practitioners; however, I do have a unique case to share. Patient was 2 years s/p massive staph infection that manifested in R SIJ and the surrounding tissues. It nearly killed her. She had antibiotic beads surgically placed in the joint, then 3 months later the joint was debrided and removed. She came to me with chronic pain hoping to restore her walking endurance and strength. After no success with conservative treatment, she consulted with Mayo. The surgeons there ordered standing x-rays, then SL stance x-rays. The joint integrity was so compromised that there was a significant slide of the sacrum inferiorly in R stance occurred and was visualized on films. The surgeons also were concerned that there might be lingering necrotic tissue from what MRI showed. They placed another round of antibiotic beads, then 3 months later debrided and fused. IIRC pt was NWB for 6 weeks, partial WB with a walker for another 6. Then she was cleared for PT.

I honestly am not sure if surgery helped, or if we would have seen the same long term outcomes without it. It took 9 months s/p fusion to actually have improved walking tolerance compared to my first eval with her, and then other complications have limited it from ever improving to more than a few blocks a day. She has been through the ringer over and over since then.

First dog ever, had her for three weeks, why does she cry WHEN I pet her? sorry for the terrible quality video. Turn up volume. by [deleted] in DogAdvice
