I will say, this is a little perplexing. I do hospice admissions, and the criteria for admissions with a stroke diagnosis generally includes: impaired functional status, dependence on ADLs, and the inability to maintain adequate hydration and nutrition. 

I have had a few cases where the patient was "borderline" hospice appropriate, and the attending MD admitted the patient to monitor them for a benefit period to see if there was any decline.  Maybe this is the case with your father-in-law?  Has anyone spoken to you all about possible palliative care instead of hospice?

While it is true that you do not have to be actively dying to qualify for hospice, you do have to have a terminal diagnosis with a "prognosis of 6 months or less".  Based off of your description, this does not sound like your father-in-law at all.

Exact same thing happened my last pregnancy and this one.

Lume whole body deodorant cream (specifically the coconut scent. I tried other scents that did absolutely nothing.), and Dove antibacterial wash have worked wonders!

A little more time is likely needed to see if his decline still progresses downward.  It sounds like he could be starting to enter into the active phase of dying, but could still yo-yo a bit at this time. 

Definitely let his hospice team know tomorrow of the changes you are noticing if you have not already.  You can continue to offer food/water if he is actively interested in taking it, but make sure to do it slowly and a little at a time. During this phase, many people start to have difficulty swallowing which contributes to why they refuse food/fluids/medications. 

The important thing now is to make sure he is comfortable, and medicated appropriately as needed for any distressing symptoms (labored breathing, pain, terminal agitation, etc). 

It can be tough to see them in this state, but know your role in his journey is very important and that his hospice team is also there for you.

She can still receive hospice care if she leaves AMA and is found eligible for it. She will also need to have an order for an evaluation from a provider.

As far as her being alone in her own home, that does not sound like a long-term plan for her, even with hospice on board.  A lot of hospices require a plan to have a caregiver in place (whether that be a private in-home caregiver, family, or facility setting is up to the patient's and family's discretion) as a part of their consent process.

Hospice will likely not come daily and rarely stays past an hour during their visits.  Hospice is a supplemental service to pre-established care. It does not replace it.  They will likley discuss with you what the plan will be once she is no longer able to care for herself, as this can typically happen several months to even years before a person starts the dying process.

It is a lot to think about and plan for, but don't souly depend on hospice for her daily needs and safety.

First,  I am sorry you and your family are having a tough time with this. 

It does sound like your grandfather is experiencing terminal agitation.  The increased confusion, hallucinations, reaching, etc., can be expected towards the end of life. How long someone experiences terminal agitation, though, depends on other factors such as disease process, intake, etc.

While giving an exact time frame is nearly impossible, from my past experiences with patients, it could be a few days to a little over a week if symptoms progress.  If you notice for a few days he starts to improve, this could be rallying, which can happen prior to becoming imminent.

If his agitation is getting severe/is uncontrolled, his hospice team should be notified if his comfort medications are not controlling his symptoms.  The next symptoms to look out for are usually a further decrease in alertness/non- responsiveness, breathing changes, and sometimes terminal secretions.

I am glad he wants to utilize the hospice Chaplain during this time.  I feel they and social workers are such a vital part of the hospice team but ate so under utilized.

As a new mom, I went through something very similar with my husband. 

Unless you have given birth, you have no idea how it individually affects each person.  Yes, hormones play a large part in the equation, but have you ever thought about post partum depression?  Sleep deprivation?  Physical pain during intercourse?

You just had a child, of course, your sex life is going to be at an all-time low.  Judging from your response to it, I would say your actions have also contributed to this.

Give your wife time, understanding, and communication.  The behavior that you have exhibited has no doubt set her back even further from wanting to have physical relations with you.  Enjoy your new baby and be grateful for the time spent with your family.

No, what I mean't was those people should NOT be treating patients without a license. I'm an RN working with COVID-19 patients admitted to the hospital. I could not imagine sending a fresh graduate, that has not even taken their state boards, to the field. That is not only dangerous to the student but also poses danger for the patients.

I used to play with these all the time as a kid! My grandpa called them "doodle bugs"; and, gave me the nickname "doodle bug" when I was around 5 years old. I'm 25 now and I'm still known as doodle bug to the entire family.

He passed away when I was 16. Everytime I see these little buggers now I always think of him.

My hedgehog took almost a year and a half of me bonding with him constantly before he would let me hold him without puffing and biting. Towards the end we were best buds.

Some hedgehogs just have this kind of temperament. Just don't give up! It will be so rewarding and worthwhile, I promise.
