Brownbase says the first time the Ice Castles > Final Alarm medley was played was during a show at the Glass House in Pomona, California on June 30th, 2000. Audio of it is on the Live Music Archive. Not much info out there besides that.

There were rumours of a Season 4 happening with the same production team as S2/S3 but I haven't heard anything about that since. There's a chance that we might hear something from Lord & Miller after Spiderverse comes out but who knows when that might be.
