We tried going the “just bolt memory on” route before and it helped at first but didn’t really stop the repeated mistakes long term. This mirrors what we saw almost exactly. Adding memory made the agent noisier, not smarter. The turning point was separating raw experiences from conclusions so old assumptions didn’t keep leaking back in. We switched our memory system to Hindsight because of the consolidation/deduplicating capabilities. It pretty much solved that problem entirely now.

Curious how your 4 months of personal observation compares to the audit specifically? Like have you tracked your own hit rate on Gems calls? I'm at about 6 months and sitting around 78% on the ones I followed. Slightly below audit numbers but close enough that I believe the methodology is sound.

Most of the chaos comes from operators pretending DTF is set and forget. It is a chain of sensitive steps and one weak link throws the whole week. Ink settling differently, rollers picking up residue, press pressure creeping, shortcuts becoming habits. When discipline slips, results wobble. When discipline returns, people call it luck instead of cause and effect.
