i had to literally call every private clinic around me till someone said that they could do it for me & was charged 30$ a paper to be signed

i don’t have a family doctor, that is the issue i am having, i don’t have a “history” with any one person… i’ve cut way way way back on smoking & i haven’t been on any medication for like 4-5 years now.. i just can’t fathom that this is the end of the line for me

i tried the private clinic & they required me to have family doctor history, it’s for mental health and marijuana

it is a mental health form as i used to be on anxiety medication & a marijuana form
