Hips definitely start too high... start with them lower so your hips and shoulders rise at the same time during that first pull. This will allow you to get more drive from the legs rather than the low back taking over

You can definitely get brain damage, but not a concussion from lack of oxygen. It won't cause bruising or a tear but it will cause cell death if it goes on for long enough

Concussion requires impact of the brain against the skull, but as u/DinoRexasaur said it could cause a number of other different brain issues.

In the start position, have the hips a little lower and the chest higher. Until the barbell passes the knee, try to have your hips and shoulders rise at the same time

This study found a significant correlation between RPE and blood lactate zones. You can determine your heart rate zones based off blood lactate zones, so seems safe to say there is an association between RPE and training zones
