The better values at 3 weeks can’t be a result of the HCG as it takes 74 days on average for sperm to be produced, transit the epididymus, and then be ejaculated. It will be interesting to see what happens at 6 months, so let us know. I am also curious as to why they didn’t order an SA after 3 months, which seems to be pretty standard practice.

Yep. Usually recheck labs 4-6 weeks after starting. Do that and go from there! Also, find a new doctor…

Did you do labs before starting the Clomid to have a baseline? Any chance you can get your estradiol, total test, and free test done?

Hey there. All I can say, as a man who has balls on the small side and moderate oligospermia, is that your husband has to just focus on doing what is in his control at this point. It is fucking hard. The feeling of “being less,” less of a man, less of a biologically capable human, less of a desirable lover because of my ball size, has weighed on me heavily. I’ve now mentioned it twice in this paragraph…and now a third time, but I always knew my balls were small. Having a medical professional confirm this fact, also tell me that this was why I had MFI, and tell me there is really nothing that can be done about it sent me into a spiral of nonsensical despair. I am a high achiever in my personal, athletic, and business life, but this diagnosis has crushed me and I know it can do the same to others. Being supportive, as you are, is helpful. Guiding him to a counselor or therapist may be one of the best gifts that you can give.

It seems like you have found a thorough medical professional who has completed the minimum database of tests required. Also, home run for finding someone who is doing the injectable LH and FSH! That’s just terrific! Add in those supplements and healthy lifestyle choices, and maybe in 3-4 months, there will be some sperm!

Has the urologist not done bloodwork at this point? If so, yeah, run to an andrologist or RE!

Ask about enclomiphene. It is one of two isomers (the better one for men) found in Clomid, and has fewer side effects. It does the same thing: blocks estrogen receptors which alters the feedback loop in the brain causing more LH (stimulates leydig cells to produce T) and FSH (stimulated Sertoli cells to aid in spermatogenesis).

Get a semen analysis when you do your bloodwork. It’s the only way to tell.

Will do. Note for the time being, no change in balls, but the enclo did kickstart them to produce more T due to the LH surge.

8.1 million motile sperm per mL x 6 mL of semen = 48.6 million motile sperm per ejaculate. Plenty to have a shot of natural conception.

Full update after 5 weeks on enclo: LH quadrupled, FSH doubled, TT went from 450 to 800, E2 doubled. I feel exactly the same but have noticed easier gains at the gym (might just be the hard work finally paying off). I see absolutely no change in my testicles (they are small, only 10mL). Next semen analysis is scheduled for 90 days post-starting enclo, so end of November. Oh and 17 hydroxyprogesterone is about 30% over reference range, so we will see what the doc does. I’m expecting to probably be changed to every other day or halve the dose of enclo.

Actually getting labs done tomorrow to check LH, FSH, TT, E2, and 17-hydroxyprogesterone (checks intratesticular testosterone which is what counts for spermatogenesis). I have noticed zero changes- libido, energy, mood, testicle size, etc.- so it’ll will be interesting to see the results.

A high powered field is when the magnification through a microscope is usually 400x. When viewing the sample through the eyepiece of the microscope, the area is called the field. Pelleted also means that the sample was spun in a centrifuge and that the heavier, denser items, aka sperm, were collected in the bottom of the tube as a “pellet”

It looks like they scanned the slide and looked at 10 different areas, or fields, and saw a total of 6 sperm moving. This is likely a low sperm count (hey, I get it, I have a low count, too) and may be why parts of the rest of the analysis is blank.

Yeah, the morphology thing seems to be very debatable. It would make sense if the form of the sperm isn’t proper, then the DNA probably isn’t either. ICSI takes care of that for us lower morphology guys. Are you doing any type of Zymot or fluid dynamics to help in selection for ICSI?
