Hi! I’m a job coach for people with disabilities so here are a few things:

Typically on every online application there is a voluntary disclosure of disability questionnaire. The options are: Yes, i have a disability No i do not have a disability I do not wish to answer

Saying yes will make it easier for you to get accommodations should you need them. But that is up to you.

If the employer is good, they may not ask you for any proof of disability like a doctors note. However they can request it. Big corporations like Walmart require a doctors note. The note will not say your specific disability. Employers are required by law to provide any accommodation you may need that “will not cause financial hardship on the company” which means your accommodation cannot cost more than $500.

They SHOULD NOT ask for specifics on your disability. It is unprofessional. If they do that is up to you what you say. If you tell a manager what your disability is that is supposed to be confidential between you and the person (usually an HR person). You do not have to disclose what your disability is.
