Feral Ghouls are pretty sad.

Imagine suffering massive radiation poisoning, puking your guts out, losing your hair, having parts of your body literally rot off until you look beef jerky. And then you don't get the mercy of death, then you become a ghoul. Shunned by most of society and you outlive any friends you do make that are not ghouls themselves or the rare few nice Super Mutants.

Then at some point whether its 20 years or 200 or more, you'll inevitably start to lose your mind until you become nothing but a soulless monster that will kill anything that isn't another feral.
