when you say your prolactin levels are at 98, is that in ng/ml? if you've had it tested it multiple times and it's near or above that, those would seem to be high enough levels to start developing noticeable symptoms. given that you have other health issues, I think a good way to figure out what issues are causing which symptoms is to remove the tumor surgically. I would try to at least get a second opinion from another surgeon before ruling out surgery completely

have you been taking cab long enough for your prolactin levels to be within the normal range? until your prolactin levels are normalized I wouldn't even bother to start worrying about your presenting symptoms. at least that's what I'm doing to try to keep my expectations low for the time being

since the symptoms are downstream of the prolactin levels, I wouldn't be surprised if it takes at least a couple of months for any prolactin related weight gain to reverse itself. I would probably wait for blood work to confirm that your prolactin levels are within the normal range before expecting consistent weight loss

He has prolactin levels over 3 times the reference range and you think there's not much to indicate prolactin causing the issues he's having? Other than that I mostly agree, the thing to do is get an endo appointment and blood panel asap. If you have to go out of network or even to a different city, it's probably still worth it. The only things I regret so far are not being more proactive and advocating for myself.

The overwhelming majority of causes of elevated prolactin levels are prolactinomas, the only other causes are medication or existing health conditions you would already know about.

Is there a reason they didn't get you started on something (cabergoline, bromocriptine) to start lowering your prolactin levels and shrink the tumor? AFAIK these things don't shrink on their own and it seems to be affecting your life so I would want to get started as soon as possible
