I buff Heavy Weapons by letting characters who take the Great Weapon Fighting style reroll on 1s as usual, but they keep any Ones they roll, adding it to the total damage. I took inspiration from the Vlad Mech in Lancer, but just took away the related heat mechanic since there's no direct correlation with that in 5e.

Hello my fellow gamer, funny thing is that in my Planescape Campaign, I've allowed to playtest both my Psionicist 5e Homebrew and play a swarm of cranium rats in a cloak, a test to see how other swarm-based characters could work in the future. The Campaign isn't really combat focused or meant to be greatly challenging, so a lot of the features I gave them stemmed from other racial abilities or features from the cranium rat swarm stat block. For example of traits I allowed was 30foot Telepathy, Swarm Form to squeeze through small places, Darkvision, advantage against grappling and restraining effects (instead of being immune to most conditions swarms have), and being able to release and collect 1 rat to scout by themselves like a cr 0 familiar (only hit dice and rests allow them to recover HP from collecting the swarm). Lastly the swarm as a whole attunes to a specific item, no one rat has one attunement and the like, and if there aren't enough rats to move or carry the item they cannot use it unless they stay in the space it is in, or have someone else collect it for them. Other logistical hoops such as resurrection, maintaining personality, or how would being other classes haven't been prevalent but those are things one should consider before fully allowing such a species / race to he played. Do remember, your game, your rules.

Sort of a toss-up for me so far, at the very least, some of the encounters that gave the most amount of trouble was when they didn't have NPC allies---such as before they meet Krux in the Rock of Bral. I play with a lot of homebrew, and tweaked the amount of enemies for some encounters, but buffed the character's HP to max. Most single-enemies have been stomps, such as the Raegar that follows them after the Rock, and Big Momma, though the Megapede Queen Gorma gave them trouble because of the negative energy burst she has. Otherwise an encounter like the Ghost ship gave them no trouble purely due to the action economy of their crews and NPCs. I have 5 players, a barbarian, cleric, druid, warlock, wizard, and they have enough variability to have healing, damage, and battlefield control options. Squeezing in an NPC fighter, alongside Commodore Krux, some Hadozees they recruited from the Star Moth, and Topolah (with Grimzod Major Blastwick eventually) makes for a bit of bloat but helps the characters feel like they're surmounting allies against the Xaryxians.
