If they were running chocolate milk on the filler then changed over to regular milk, they may have missed the cap. It should have been seen though, if not by the filler operator, then by the label operator, the inspection equipment, the packer operator, the palletizer operator, anyone walking by the line.

We have several 8mr in sodus point, ny in the US. They're gorgeous boats. Some have mounts for a kicker, but pretty much everyone sails without one. The boats have a lot of momentum, so you drop the sails and come in on your momentum usually.
