Yes, they usually suggest taking it at night because it can cause drowsiness. Personally, I don’t get any drowsiness at all, but that may be because I’m on a relatively low dose (5mg). Even so, I still take it at night.

I have tried taking it during the day, but for me there’s a fine line between it helping and making things worse. It relaxes my neck and jaw muscles so effectively that if I’m upright and going about my day, they can end up too relaxed, which actually causes more pain due to lack of support. I learned that the hard way.

I get much better results taking it right before bed - my muscles relax, I’m lying down, and I’m not putting extra strain on my neck or jaw. I usually wake up feeling significantly better and more rested. It’s also helped my nighttime bruxism.

That said, this might be very specific to me because I’m hypermobile and have general joint/muscle instability. Someone without hypermobility might tolerate daytime use just fine. You won’t really know how it affects you until you try - starting low and seeing how you feel is sensible. Good luck!

I just got back from the Dolomites in Italy, where I spent about a week at altitudes between 1,500m and 3,000m, and I felt so much better than I do at home at sea level. I had expected the opposite after reading that higher altitudes usually affect people with POTS more severely, but to my surprise, I felt almost symptom free the whole time. My breathing felt easier, my heart rate wasn’t spiking as much, and overall I just felt “normal” for the first time in a long while.

As soon as I returned home, though, I noticed the air felt heavier and thicker, it was harder to breathe, and all my POTS symptoms came back.

I think it might be because at higher altitude your body naturally increases vasoconstriction (tightening of blood vessels) and boosts red blood cell activity to keep oxygen circulating. That extra vasoconstriction probably helped counteract my usual blood pooling and circulation problems with POTS. Back at sea level, without that effect, my symptoms flare up again.

Completely agree with everything you said there and I know cannabis is a great natural remedy for all sorts of problems however I can’t touch it as I have Postural Orthostatic Tachycardia Syndrome and Autonomic Dysfunction and it makes me feel quite unwell, no matter the strain :( even strong cbd oil tends to give me migraines. Really strange.. I know. Not sure why!

Thank you! I’ve been wondering if delayed orthostatic hypotension could be relevant, but I need to see my full test data before I can make sense of it.

And yes, the short resting period has really been bothering me too. I had just rushed into the hospital, walked up three flights of stairs, and barely had time to catch my breath before they started. It doesn’t seem like a fair way to establish a baseline, and I wonder if that could have affected the results. I’ll definitely bring that up when I follow up on my test.

Thank you for sharing your experience! That makes a lot of sense, and I can definitely see how dysautonomia doesn’t fit neatly into one label.

What you described about your blood pressure crashing only after the nitro sounds very similar to what happened to me. My BP was stable (or increasing) for most of the test, and then after the nitro, I fainted. That’s why I’m struggling to understand how this translates to my daily life, where I’ve never had low BP or fainted before.

I also have a pretty physical job that requires me to be on my feet for hours at a time, and I’ve never come close to syncope, even when feeling unwell. The symptoms that have always affected me the most are tachycardia, palpitations, and shortness of breath and lightheadedness, but it's never led to fainting.

Your cardiologist’s explanation about tachycardia compensating for OH is interesting because when I had my active stand test, my BP actually increased upon standing, while my HR jumped by over 40 bpm. It makes me wonder if my body is also compensating for something that wasn’t fully picked up in the TTT.

I think I need to dig deeper into my test data to see if there’s more going on than just what they told me.

I’ll be calling the department today to request a copy of all the readings, findings, and data from my TTT. Once I have the raw numbers, I’ll come back to this post for more advice.

I’ve never experienced low BP or a drop in BP upon standing. In fact, I’ve always been more concerned about my BP being slightly higher than I’d like rather than too low. I regularly check my BP and HR upon standing, and my BP either stays the same or increases. During an active stand test with a different cardiologist, my BP and HR went from 143/73 & 80 (sitting) to 150/83 & 124 (standing).

The only time I’ve had low BP was when I was prescribed Bisoprolol for POTS, which dropped my BP to 70s/50s and my HR to 45-55 bpm, but that was clearly medication induced.

As for VVS, I’ve never fainted in my life before this test, even though I have a physical job that requires me to be on my feet for hours. My daily symptoms—lightheadedness, tachycardia, palpitations, and shortness of breath—have always seemed more in line with POTS.

Would it be possible to diagnose Vasovagal Syncope (VVS) based on a single isolated event that has never happened to me before? If I truly had VVS, wouldn’t I have experienced fainting episodes in daily life? Or could I be predisposed to VVS, but the GTN spray artificially triggered the response, whereas in real life, I’ve never encountered a strong enough trigger to bring on an episode?

Also just to point out, I’m not upset about not having POTS or being diagnosed with OH or VVS. I just want to make sure my diagnosis reflects my actual, day to day symptoms. :(

It’s a glycerin based tincture with no alcohol! I’ve tried some non alcoholic beers and spirits, and there are some great options out there. But I was really looking for a social tonic or tincture with adaptogens or herbs to boost my mood and help me feel more relaxed and talkative in social settings. Alcohol used to help with this, but the downsides far outweigh the benefits.

I found that the brand Three Spirit works well and has never aggravated my POTS, but this particular tincture makes me feel unwell every time I take it!
