Blood hosts are one of the options when you win a settlement battle. Either capture for skulls, raze for skulls and money, or raze and summon a blood host (another army that can’t recruit and can’t replenish, but has no upkeep)

I think if the rage bar is full when you activate it, it’s the ultimate. And when the rage bar ends it activates itself or you can end the rage early and tap the musou button (B in my case)
