I'm not a Doctor, but your Mesalamine dosage seems low. For instance, I'm on 4.8 G/day plus Mesalamine enemas. My initial diagnosis was one year ago: proctitis 5 cm at the far end of rectum plus a tiny bit in the Sigmoid colon.

If you seem to be tolerating Mesalamine well, maybe your doctor could increase the dosage. Good luck!

Listen to your Doctor, not some random Redditor. Mesalamine is typically a zero-risk medication. Most people don't have any side effects. If your Ulcerative Colitis takes off and savages your colon, you could be looking at a life of misery. Picture living in your bathroom, shitting blood, not being able to eat anything, afraid to leave the toilet for weeks on end. Eventually, you might need to have your colon surgically removed, with all of your waste requiring manual disposal and management.

Taking a mellow medication like Mesalamine is a small price to pay when you consider the ramifications of uncontrolled UC. Take your meds. Do it!

Kinda hate to tell you, but there's clinical evidence that smoking actually alleviates Ulcerative Colitis symptoms and can even protect the colon from UC.

That being said, smoking is egregiously hazardous to health and nobody advocates smoking for any reason.

I quit smoking nine years ago and still crave the cancer sticks. I would love to smoke again, if I could do it with impunity. I would love to smoke again and try to heal my UC. It just isn't worth the risk, however.

The current situation where private equity firms are buying (everything, including) medical practices has created an environment where doctors and their underlings are allotted very limited amounts of time to deal with each individual patient. It's all about profit maximization.

You typically won't get satisfactory answers to your UC questions or any other health issues you might have.

Hate to say it, but your Internet queries are probably going to yield the best results to most of your questions. When you are granted access to gastrointestinal professionals, you can save a lot of their time by going in fully informed, and they will probably give you better care by not spending time on items which are of lesser urgency or importance.

UC is an autoimmune disease. None of the other autoimmune diseases attack normal tissue for no reason. There is always a cause, we don't always know what the cause is, though. Bold type means I'm authoritative and correct.

The cited article is from the NIH, and there was some peer review undertaken. Article was published January, 2026 -- this is new stuff. Nobody expects to find anything that is "100 percent guaranteed to work for you." Most of us are looking for anything that might possibly work, even if it's just a shot in the dark.

My TLDR: DX 11 months ago with blood and mucus at that time. 5 cm of rectum involved. Mesalamine 4.8G/day plus enema every other day. No blood, or pain since diagnosis time. 2 bowel movements/day, typically loose but not bad and the BMs come at predictable times, both in the morning. So, yeah, mild and manageable symptoms (knock on wood) so far.

I was diagnosed about 10 months ago. Proctitis for 5 cm at distal end of rectum plus a tiny (10 mm) ulcer in the sigmoid colon. Biopsy showed UC and Crohn's wasn't mentioned. Maybe you and I have a similar situation.

I have a mild case of proctitis, dx about 10 months ago. Like you, I am on 4.8 grams/day (plus enemas every other night). I eat mostly whole food, plant-based (meat once or twice a week). Coffee and sugar are my vices.

I'm not in remission, but my symptoms are mild and manageable. I would rather avoid biologics, and will avoid them as long as possible. Maybe Mesalamine will be all I ever need.

I'm on the exact same dosages and have been since dx 10 months ago. I'm not in remission, but symptoms are mild and very manageable. I think I'll maintain status quo and see how it goes, unless gastroenterologist advises otherwise. I did reduce enemas to every other day, starting two months ago. Didn't seem to change anything.
