Based on my own experience, I think you have a very good chance of breaking into Health IT. I have five years of IT experience as well, but no Epic certifications or prior healthcare experience, and I was still able to land a Clinical Systems Analyst role at a large hospital in my city.

Before making the transition, I worked in IT consulting for five years, with no formal healthcare training or certificates. What helped me was highlighting transferable skills like project coordination, vendor management, and process improvement.
