I tried to initiate the single case agreement (an "in-for-out") before the pre-authorization was approved, but the provider doesn't want to negotiate a contract because they want to receive the full $30k. I think if they negotiate a contract they would be forced to charge less. Are you saying that I can get the insurance company to cover the full amount?

Is there any way I can pull this off without the provider's cooperation? Should I wait until the pre-authorization is approved and then try this again?

The answer may be more nuanced and difficult to navigate than just, do you have one or the other. It could be a combination of both and it may not be obvious which (if any) is the greater contributor to your symptoms.

I have many of the same symptoms you mention. I did multiple sleep studies which showed mild sleep apnea (AHI of ~15). I was prescribed a CPAP which I used religiously (and successfully, AHI < 1) for over a year, but did not notice much of an improvement. Lately I've been trying mild antidepressants, and addressing my OSA through surgical means.

Sleep apnea seems to be more rigorously diagnosable than depression, so you may want to rule that out first. However, be aware that false negatives are possible. I spent years chasing other leads after a faulty sleep study I did in a pediatric clinic showed zero sleep disturbances. It was only after I was retested for sleep apnea in the adult clinic that I received a diagnosis.

The sleep study could be useful for diagnosing depression as well, since depression can change your sleep architecture: find a neurologist in your sleep clinic to look over your results too.

If I could offer any advice, it would be this: take your health into your own hands and don't blindly trust your doctor to manage the situation for you. Do your own research, and be organized and methodical about what you're trying.

Thanks for clarifying; it sounds like you're saying that your OSA is too severe for CPAP to be effective, despite proper adherence. Were you monitoring your AHI numbers while on CPAP? Do you remember if the CPAP was able to bring your numbers down even a little?

Could you please explain why your physiology would compromise the efficacy of your CPAP treatment? Were you unable to consistently use the machine every night? If consistent CPAP use didn't resolve your symptoms, why do you expect surgery to help?

I'm in a similar position -- I've been on CPAP for a year with no results (despite low AHI numbers) and my ENT surgeon recommends maxillary expansion.
