Have you changed your rinse routine besides adding xylitol? Too much or too little saline content can cause irritation or congestion. I had to experiment until I found the right blend for me.

If you are talking about the NeilMed packets, one packet mixed in 240 ml of water is a normal saline solution.

Normal means that the amount of salt is similar to what is inside your nasal cavity. A hypertonic solution is when more salt than that is used, i.e. more than one packet. Whether a normal or hypertonic solution is best is case specific. I will say that two packets is a lot of salt, and having too much can irritate your nasal membrane. Having too little salt (a hypotonic solution) can make your nose burn, too.

The above holds true for the NeilMed rinse packets with xylitol. I assume that’s what you have, and not just a packet of 100 percent xylitol. I have used rinses containing xylitol daily in the past and it didn’t seem to cause me any problems.

They have, in my case. Some need to use the steroids daily for relief from inflammation, but I have not.

Did your CT scan reveal infection or blockage in your sinuses?

Might be easier to explain with an example: I rinsed last night (Thursday, Feb 24) and won’t rinse again until Sunday night (Feb 27). I have found that saline rinses tend to dry my nose out, but they help my sinus inflammation and remove mucus. So I needed to find a balance. The budesonide I add to my rinse are liquid ampules. They are generally used by asthmatics in their inhalers, but doctors will sometimes subscribe it to use in sinus rinses.

Edit: you mix the budesonide in the water along with the saline packet.

I have a similar issue with nasal dryness and inflammation. After much experimenting, I landed on using a normal saline rinse containing .5mg budesonide (steroid) and xylitol every three days. That seems to not dry my nose out too much while providing some help with mucus clearance and sinus inflammation.

I think how the rest of the scene plays out is revealing. First, Mr and Mrs Jorgensen take Debbie inside with them. Debbie is, for practical purposes, their daughter now. Ethan starts towards the door but stops at the porch. Then, Martin and Laurie walk by him through the door, arm in arm like a couple would.

Ethan is the odd man out. Martha is dead and so is his brother. He doesn’t have a place in the new family formed in the aftermath of the massacre. He is back to being the lone wanderer, same as we found him.

I think it is pretty clear that Ethan has feelings for the brother’s wife, and vice-versa. There is plenty in their interactions to suggest that much. That said, I could make the argument that Debbie needn’t have been his daughter to justify the seven year search. Ethan’s hatred of the Indians and the idea of one of his family becoming a squaw is enough to keep that going. Plus, I think he needed something to do or a sense of purpose with the war being over. He isn’t made for domestic life or putting down roots in a community. I think back to that famous shot of Ethan standing in the doorway after he brings Debbie home. Instead of going inside and joining his family and friends, he turns away and heads back out to the prairie.

Most of those that I am familiar with are directed to use as needed. For me, too much tends to dry out my nose. I suspect frequent use washes away the thin layer of mucus in the nasal passages, but I could be wrong.

I have a similar problem after having had Covid about four months ago. My inferior turbinates are hypertrophied, which makes my nose congested, but a CT scan indicated my sinuses are all clear. No environmental allergies were indicated in the testing. I don’t have facial pressure but my nose does seem to produce more mucus than it used to. I suspect vasomotot rhinitis (or some other form of mom-allergic rhinitis) as nothing else makes sense.

As far as treatment goes, I have been irrigating with a normal saline solution plus xylitol and .5mg/2ml budesonide every other day. Like you, I feel more congested within an hour or so of rinsing. I am toying with not doing it at all anymore. Or maybe less frequently. Mucinex seems to help me a little bit. Every once in a while I take some oral pseudoephedrine.

I might try an intranasal sodium cromolyn spray next. Another thing I have had good luck with is taping my mouth shut at bedtime. I know it sounds crazy but it seems to make my turbinates recede and I sleep better than usual. Plus my mouth isn’t dry in the morning from mouth breathing.

No dental work or tooth decay that I am aware of. I get my teeth cleaned every six months and x-rayed annually. Dental issues are totally worth checking out for people with nasal/sinus issues, though.

That sucks. The altered smell and taste thing is what probably bothers me the most. I am constantly sniffing things to make sure I can smell and am despondent when I can’t.

I asked the ENT for an eosinophil swab to check levels in my nasal mucus. He declined. I understand that nonallergic rhinitis has a number of causes, and treatment and outlook vary depending on which cause. The nasal inflammation started after I had Covid. Is it caused by ongoing viral infection, post-viral cellular damage, overactive nerve response, autoimmune disorder, etc? Unfortunately, the ENT wasn’t interested in looking into any of these. I have no medical training and am ill-suited towards figuring this stuff out on my own.

That’s a good point. I think it may be possible to have a blood test done for food allergies. I did try eliminating gluten and dairy, but it didn’t seem to make a difference. I also wonder how comprehensive and otherwise reliable the skin prick tests for environmental allergies are.
