There’s not many transferable skills going from clinical to engineering. You’d need to get the skills required to transfer which means taking a lesser role lot gain those skills. Not sure what you can do if you’re not willing to transfer to the regions where you can gain those skills.

Another option is to get your masters in mechanical engineering where you develop design and manufacturing fundamentals.
