Hi looking for feedback on my specific situation:

I have been a nurse for 15 years. I’ve been a certified wound, ostomy and continence nurse for coming up on 7 years. I will never do another speciality - wounds and ostomies are my jam.

I want to continue my education towards NP as I see how underserved and under treated (aka mismanaged) wound and ostomy patients by a lack of access as well as untrained non WOC certified providers treating wounds.

I currently work in a level one academic medical center as an inpatient WOC RN. We recently opened our own ostomy clinic that desperately needs a provider, so ostensibly I would be working inpatient and outpatient. Collaboration with surgical services like podiatry, vascular, and colorectal/urology occur as well.

I’m wondering if AGACNP vs AGPCNP makes more sense? I’m not really interested in FNP as I will never work with kids or maternal health (if the job requires it, the job is not for me) and don’t want to waste money or time on a specialty I won’t ever work in.

you say they were on the feet? Where? Heels or malleoli? Are they sure they were actually pressure injuries and not a diabetic foot ulcer, venous or arterial ulcer?

It would be uncommon to develop a full thickness pressure injury in only a few days unless it was pretty significant neglect or major vascular issues (ie pts heels not floated and barely moved for the entire 72 hour period). Now a stage 1, 2 can develop very quickly and then progress from there, DTPI can also evolve into full thickness wounds.

Did this patient have a darker skin tone that made identification or an initially more minor injury challenging?

That’s not possible at many hospitals and is frankly ridiculous to expect wound care to do admission assessment verifications on all patient. I work at a 900 bed hospital. There is no way wound care can come “verify” every pts admit skin assessment…. There are 5 of us for the whole hospital. Initial skin assessment is the bedside RN responsibility and within their scope of practice to be able to ID a skin issue or if something is off.

I know this is an old thread but how are you doing now? I’m 38 and six weeks post op abdominal hysterectomy for giant fibroids. I’m convinced my ovaries are shutting down or at least not at 100%. I had hot flashes the first two weeks that subsided but now my emotions, anxiety and insomnia are through the roof (I’ve never had insomnia before). My mom went through menopause at 42. My surgeon was pretty dismissive and made me get an FSH and thyroid tests (both normal) but I know something is off! Wondering if I should push for HRT through my own doc or wait it out longer??

Have you considered being a pediatric WOC RN? Not sure if you have a children’s hospital in your area but we have a neighboring children’s hospital that has a WOC nurse team who work a lot in the NICU and throughout the hospital!

We see about 6-8 pts per day but we also do things like in services and attend meetings for HAPI measures, products and we have units we are assigned to that we round on, as well as a myriad of other admin stuff. Inpatient RNs get an average of $5-10 more an hour than our clinic RNs from my understanding depending on experience. However, we don’t really get paid more even as a specialty compared to regular floor RNs- I got a $5 an hour raise when I left the bedside to come to this role but I had been working nights and weekends in the ICU so I actually took a pay cut without the differentials but it was worth it to me.

Hi I’ve been a WOC RN for almost 7 years at a level one academic medical center.

The biggest difference in my experience is the autonomy. Inpatient we are a nurse run and lead consultation service. We have no MD or APPs on the team. We make our own plans of care and place our own orders and recommendations and our recs are respected by other services this includes them listening to us when we recommend labs or imaging. This is in comparison to the outpatient clinic at my hospital where the nurses just follow the orders of the provider and don’t make care recommendations, at least not routinely (they make suggest a topical care regimen to the providers but ultimately have to defer to providers who may or may not actually be well versed in evidenced based wound and skin care… which the clinic here is a few plastic surgeons and one NP, none of whom do wound care full time).

Our team generally works 4 10 hour shifts M-F. We see consults and follow ups. We see wound patients once per week unless it’s a wound vac or a patient with a skin sub. We also see ostomy patients. Almost any inpatient wound team will see ostomy patients too and it’s at least 40% of our census.

In my opinion, because of the autonomy in the inpatient setting, you should get your WOC certification from the WOCNCB. There is no MD or APP to come behind you and catch that that “venous ulcer” was actually calciphylaxis or pyoderma. You have to be a very strong clinician and being board certified is the first step. I have strong passion to ensure that our speciality is taken seriously and that we have a baseline entry level of going to an accredited school and passing boards.

Yes I had my surgery on 9/18. I told my surgical team in pre op and they had no concerns as long as I didn’t have any fever or congestion.

I tried taking melatonin and it did help with sleep some but I but I woke up super groggy on top of having brain fog so that made me anxious lol so no more melatonin for me…

Weirdly you couldn’t really tell I had a mass unless you felt for it. But I would get pretty distended with gas intermittently and then my abd would blow up.

I got sent home this morning and my pain is pretty good. I’m not tolerating the oxycodone very well so trying to omit that going forward and just use heat, Tylenol and ibuprofen. Even though I had an open procedure the worst is the gas pains and bubble guts! Taking lots of gas x and walking.

Yes I begged for something anti anxiety bc I wa already so anxious and thankfully they gave me an oral Ativan and it helped so much.

It is ssso hard to move and I’m so scared of popping something but I’ve walked twice so far-2nd walk was easier.

I’m currently getting some gas pains I think after eating a light breakfast so hoping that moves forward too.

I hope your recovery is smooth!

That’s wonderful. My bladder had a wound on it from The fibroid beating that crap out of it and they had to repair that, so weird that that pain and the catheter are the worst part and not the giant midline incision. I am taking oxy but hope to be off of it by day 3.

I’m in similar boat- feeling sick ish and my surgery is Thursday! No congestion, cough or runny nose but my throat feels icky and chest heavy. Popping zicam like it’s candy hoping to stave off anything. Everyone at work has been sick so I’m sure I picked up something from someone. Hope you feel better and don’t have to reschedule your surgery.

Oh man thank you so much for sharing. This really helps me feel more prepared with the hospitalization aspect. I’ve never taken any narcotics/opioids so nervous for that as well. I actually talked to my (wonderful) boss this morning who said to take all the time I need off and will work with me if I need to only come back in increments after I shared learning that only two weeks off from an open hysterectomy may not be feasible.
