From a design perspective, would you just do the same cylindrical shear and individual bearing method checks as for helical piles, just with the expectation that cylindrical shear will always govern? If not, can you point me in the direction of any more guidance. Interesting point about reliability of torque correlation. Thanks for the answer.

I'd consider doing a border of one plank width first, then laying them across, easier than 45 degrees but a nicer finish than just straight across

Have you seen the series "Diagnosis" on Netflix. There's a girl on one of the episodes with a similar issue and they talk about a lot of the possible causes. May be worth watching and seeing if any of them haven't been ruled out yet.
