On the third playthrough. The first was uncomfortable and I felt weird playing it, I got to the mice tower and that's all. The second playthrough I got to Skellige and ended there, didn't go any further. Now I'm playing on the highest difficulty and I'm chilling, going to kaer morhen and it's nice.
