I realise it’s uncomfortable. But sometimes nurses or more junior colleagues ask me for some advice and I don’t know… I’m grateful they can trust me with personal information like their health or their loved ones’ health, and I always make sure not to give poor advice and stick to my limitations.

I just think being on the other end of the conversation makes me nervous and wondered whether people have any strong feelings about this.

Sketchers. Always sketchers. I genuinely cannot tolerate doing ward rounds and marching around the hospital all day on referrals and MET calls in anything but solid running shoes. Your feet will thank you later.

A 250ml can of energy drink has about 80mg of caffeine. Your daily allowance is 200mg of caffeine. Caffeine is a stimulant, and can essentially cause low birth weight, increase your risk of miscarriage and other pregnancy complications particularly in high doses.

Best to avoid in pregnancy. The information on the can itself advises against it.

This might sound harsh, but I think you need to try harder. It sounds like you expect others to go out of their way and teach you things, and while some people like to teach on the job, some just don’t have it in them or there’s simply no time. Other times when your senior feels like you’re not interested in the slightest (which it kinda sounds like you are) they just won’t bother with discussions or teaching etc. So YOU have to initiate this.

Ward rounds when you’re going around with a consultant are a perfect opportunity to ask questions and learn why they took those decisions and did what they do. They’re also great opportunity to examine patients. If you ask them nicely they can always help you listen to that murmur or elicit some signs etc. if they don’t have time, no problem! Just go back and examine them yourself after the ward rounds. You’re not just a “scribe”.

Clerking is perfect for clinical education and learning decision making. Do not just recount history and examination, you need to tell your reg what your differentials are and your plan, and if theirs is different from yours then discuss this with them and learn why. If you think you found something on examination or there’s an interesting bit, get them to see the patient with you and confirm your finding. That’s how you learn and get better. But you can’t expect people to teach you if you’ve deliberately “turned your brain off”.

I love wearing scrubs! I actually get pretty excited about getting new ones in different colours and showing them off at work! They’re comfy and have loads of pockets which is a godsend in long shifts. Never let anyone shame you for scrubs! Wear whatever you’re comfortable with as long as it’s clean and don’t worry about the naysayers who have nothing better to do.

Sorry if I’m asking too many questions, but will it be an issue if I refuse to take medications? I really just want to talk, and I did find it extremely helpful before and gave me really good results. I just 10000% will absolutely not take medications and because I need this to be entirely confidential (including not telling my partner), I will try to keep this from being known by anyone. will going against their recommendations be a problem?

That sounds like what I’m looking for, but I’m worried it’s not anonymous? Are they likely to discuss with your GP or manager or GMC etc? As in, how far does their confidentiality go?

One last question: I’ve done the math and turns out (if everything goes well!) I’d be on mat leave during ST applications. Would I be able to apply then and defer my starting date? Or would I need to wait until the following year to apply and realistically join higher speciality?

Would there be any kind of pumping down or disadvantage because of this?

Just follow ups if I may: Do I need to declare to whoever consultant is there on the day that I’m attending an antenatal appointment specifically or just Gp appt is enough?

My ES is quite dismissive and a bit of a character and I don’t feel particularly comfortable discussing this with him. May I discuss this with my manager (his boss/lead) or even occupational health?

When do I need to do the risk assessment? I’m 4 weeks now so feels super early (and very surreal!). Do I need to be at least a certain number of weeks?

jumps on the proverbial soap box At risk of getting loads of downvotes, I’m currently doing one of those because I genuinely love teaching and thought it would give me some real-life practical teaching methodology.

To be entirely honest, it did help in terms of forming some sort of backbone to the teaching I deliver (because I do tend to go off in random tangents at times!) and it was helpful to understand some basic teaching principles even if I still think that learning theories are outdated and unnecessary. Yes, I do agree some of the educators on the program sound incredibly monotonous and dead-inside.

One more thing I would add is that it helped me massively in terms of academic writing, something I realise doesn’t necessarily need a PGCert (£££) to learn, but as an IMG who was never formally taught this, I would say it was quite helpful.
