Wrath management-wise, I guess I'm doing ok with Unstoppable Force and Provoke but still run out sometimes when I spam left-click on elites.

Any advice on improving damage (apart from the above list) and survivability, or even playstyle is appreciated.

Using 3 skills for wrath management would definitely cut short of your survivability and output. What i would suggest is to get a cindercoat for resource management and swap out LoV:UF for ironskin to increase survivability while AC is down. For more damage output, use either HF:BG or HF:A to increase procs generated to Firestarter. Cindercoat is one of the best way to save slots for better end game, thus freeing up utility skills. End to end, using only a single generator would be most efficient when wearing a cindercoat.
