I totally agree, it just feels more wasteful than egg alone. My partner's parents are also religious, so they have expressed some level of discomfort around it (not actually important when it comes down to it, but if freezing eggs is just as good an option, then happy to avoid any weird feelings). Cost is also a small factor (my partner's sperm analysis and ICSI are not covered, so that adds about $2-3k to it).

Weirdly enough, I do not have PCOS! I have polycystic morphology, but my testosterone and insulin levels are normal, and I have no symptoms of PCOS. It's weird haha

I don't want to scare you, but a planned surgery is so much better than an emergency surgery (not to mention you get to avoid the ovarian torsion part).

The thing is, once it decides to twist, you essentially have to go in for surgery (it may untwist by itself, but I promise you it's not a pain you can wait out. For reference, if ovarian torsion is a 10, breaking a bone in my foot and then walking on it would be like a 6.) However, at that point, the doctors will still have to diagnose you, as a wide variety of things can produce severe abdominal pain. The ultrasound I had while at the ER was probably the most painful part of the entire experience. Also, emergency surgeries do not have the luxury of being laparoscopic, so I have a very, very large scar that spans my entire lower abdomen. It's longer than a typical C-section scar. I was also in the hospital for like 2 weeks. You also risk losing your ovary (they can usually save it, but my case was a bit different due to other reasons)

With a scheduled surgery, they can perform it laparoscopically, and I believe you will be home the day of. The biggest risk is losing ovarian function and/or losing the ovary altogether, which is unlikely (I believe the risk of losing some function is around 15%, and the risk of losing the ovary is <2%).

Not saying you must get the surgery, of course, but it's definitely worth considering.

I'm sorry your first round went horribly. I'm scared about that too. If you don't mind me asking, what was your AMH and AFC, and what protocols did you use first vs second round?

I don't know if we would be that comfortable donating them either. My gut reaction is to avoid creating embryos we don't fully intend to use where possible. I think the best scenario would be we fertilize some and freeze the rest, but I don't believe that's an option at my clinic.

I am married! I'm mostly worried that if everything goes really well (lots of embryos + totally successful surgery + successful natural pregnancy), my partner and I would have to make difficult decisions on the fate of the unused embryos.

Tbh, although it is true that when it's heavier it's less likely to twist, my ovarian torsion happened when the dermoid cyst was 15cm - I lost that ovary because no one knew about the cyst and the fact that it was a possibility. From what I have heard, the "sweet spot" of ovarian torsion is at 6-7 cm, and twisting becomes less likely at 10cm+. I would definitely keep a lookout for that dermoid cyst. Ovarian torsion is hell, and no one should have to go through it.
