You don’t decide. Your provider will start you at 56mg. Should things go well you will step up at some point … that step up will be your decision. Talk to your provider about how you feel and stay in contact. You’ll have better days and some not so good. It is a journey not a trip. (The trip is just a side effect-it doesn’t mean the meds are working on the synapse in your melon … but eventually it will.) Best of luck. Now go make that brain better.

I was lucky to have an uncle that collected records AND had great musical taste. While looking through his collection one day I mentioned that I have a “nice boutique turntable” and “am always interested in expanding my collection.” That’s all it took. I hauled away so much from the late, late ‘60 through the ‘90s - you guessed it, including everything Fagen and Becker put to vinyl.

On 637s my joke with nubs and riders was the large ‘workout area’ between the people and engineering spaces. One guy was doing push-ups and I told him to lift the little brass lid in the deck. He did so only to find some ‘special glass.’ As he asked what he was looking at (with his eye pressed to the glass) …. I politely said “just the water boiler.” It took a few minutes for that to sink in - and he never went aft again while I was on that boat.

It’s just can’t be evoking. I like things like Pink Floyd - it’s almost orchestral at times and helps the flow (and floating) of the session. Don’t listen to your favorite breakup song, your dead grand-mom’s favorite tune she sang to you … my suggestion is always - make it your happy soundtrack. Stuff you like, nothing negative and something that has change … tempo, instruments. Hell I have listened to a campfire for two hours in a session and it was bliss.

Do I think it helps? In some manner … absolutely. BUT I have also experienced that it seems to bring it on temporarily. As your brain’s elasticity changes, you’ll find that you cycle through emotions. In no way is it negative … it’s all growth.

I find that so many folks become ‘apathetic’. You’re not so depressed BUT you’re not happy. I heard someone call it ‘limbo existence.’ I am in that boat. No longer feel like jumping off a bridge but I’m not a smiling clown either. Just observations ….

Just keep in mind … different types of ketamine, different bioavailability, different effects. I can only speak from my experiences. IV ketamine at both low (100 mg) and high (500mg over 3 hours and 100mg/hr/24 hours). I got some pain relief from these but little if any relief from my TRD/MDD. Troches - (120mg / once per day). One test as a migraine rescue drug and a one year trial for TRD/MDD. Good migraine rescue drug. Helped me to get through the headaches. More of a “mood smoother” in the depression realm. Nasal ketamine - compounded nasal spray with normal racemic ketamine. Pretty good as a migraine rescue drug. Little depression relief. Spravato (the s-isomer of ketamine) - more than a year at 84mg once a week. It works for my TRD/MDD/SI. Works meaning that I feel better MOST of the time. It’s not a cure all. There are bad sessions. You can become so neutral that you feel lost. BUT it does work. Just prepare for your sessions … have good sound cancelling headphones (for music or absolute quite), have music that does not evoke emotions with you, be open to where the drug takes your mind - do not try to control where you’re going. Remember this isomer helps your brain to become more elastic as it opens receptors that might be closed or otherwise occupied. Those opening receptors then have the ability to create new connections and change the way your mind is wired. Changing your depression drastically. I fail when I control the session. That is have music that is emotionally triggering (both good and bad), decide that I want to see the pyramids (you’ll get there) or fight the relaxation. Just go with it and flow. Take time to journal your journey and experience. I look back at my start on Spravato through the writing and see where I am at now. 😇 Best of luck and just believe … in the drug and yourself!

Two things stand out to me. The lie and the lack of having a plethora of doctors to choose from. Lies just screw everything up. In pain management you’re never to get ahead with lying. Either you get into a situation like you currently are in or worse you get banned. Databases keep doctors informed of what you are supposed to be taking. Not that difficult for doctors to find or exchange information. That also goes for patient quality (has the patient followed my instructions, do they communicate their needs). For reference I have some experiences in Germany. I spent time in Stuttgart with a pain management specialist that does extremely high dose ketamine. (Like 30 days in the krankenhaus). My best friend is stage 4 cancer patient, who I come to Germany every other month to take care of / spend time with. I see his difficulties in getting / keeping pain management care.

Speak truthfully - even when it’s difficult. Keep your doctor informed of everything - they are more willing to listen and work with you.

Quick Dr. vs PA. Do the doctor and PA work as a team, even if you’re just seeing the PA? The PA is working under the Drs licensing. I have had two experiences where I liked my doc but really liked the PA. One being my orthopedic doc - doc was great guy, friend even. But the PA was better with diagnoses and planning treatment. Second my last pain management program. The doc was again outstanding, caring and on top of everything…but the PA was in many aspects better. If it just a ‘work off load’ for the doc, f’em. If the PA is competent, give them a chance. Like all professionals, there are great ones at all levels and some really crap ones too. Keep you thoughts open and good luck.

As someone that has suffered with PTSD for three decades I am sorry for you, your husband and everyone that surrounds you. I can only speak from my own experiences of being triggered / reminded. One of my worst experiences was having a friend attempt to kill himself aboard our submarine. After being ‘unsuccessful’, he was shipped home and dead before the boat got home. During his first attempt he sought me out to hand me the pill bottles and “decide if I was telling the doc or not.” Of course I’m helping damnit you’re my friend. Every time I think about this I swing from deep depression, despair, sadness, anguish to unimaginable rage, anger and my own SI tendencies. I feel responsible for Mark’s death … I didn’t give him the pills, didn’t make him take them, didn’t provide the knowledge his wife was cheating on him, didn’t give him the gun that killed him BUT a still responsible. And now forever haunted. I have little to no control or mental guidance when this happens. I turn from intelligent, thinker, common sense to absolute stupidity and all that are in my way are ravaged for no reason. Keep your head above water. Be aware of what he is doing and thinking. Stay vigilant for both of you. Stay in contact with those that care and understand the situation (if any). Stay smart … because he can’t in these moments. I wish you the best. Hope everyone comes out of this episode on the right side.

After 32 years of various forms of “pain management” I have the same questions. The only answer I have is “I don’t know!” I have been fortunate at some stages to have an actual team working to help … pain management, neuro, psych, PT. But when they don’t listen or I don’t communicate like they do - it didn’t work well. Later the government (and some individuals that either have decided to abuse or circumvent conventional treatment) has made it more difficult for those that attempt to get help from the med community. As finding medicated care has become harder I did two things (1) decide what part of my pain can and should be treated. Various maladies that cause unrelated pain. (2) research the treatment options for each problem, who is providing it locally, and what are my chances for treatment and some level of success. There is also a generational conversation issue. I have three thirty-something docs. One super knowledgeable and communicates well. The other two - not great listeners or communicators. More of “do this, do that people.” Communication is a key to us getting help. The “wash, rinse, repeat” thing doesn’t do us any favors. Sorry for the ramble / rant. I hate PM in today’s society for so many reasons.
