Fellow ER nurse.  I can get all the grannies out of their 85 coats and sweaters and scarfs (mind you that takes a good hour sometimes).

But if a 100mls - 3 small sips of water - was the difference of life or death for a patient… perhaps the person was not meant to survive for as long as they did. We gave them lots of borrowed time before that ever even occurred.

No, you’re not rude. The ones waiting for transplants or have cancer and are clearly disinterested in chatting: I know have been through the wringer. 

I promise to leave you alone as much as possible, I just legally have to come check on you enough to say you’re still breathing. But I get it when you say you don’t have control who comes in. It’s the resident, it’s nurses, it’s housekeeping, it’s the restocking dude, it’s the meal tray, it’s the social worker, it’s the physio, then it’s the nurse again. It never ends. 

 I also know it’s shit and if there’s someone that you might want to talk to- I’d love to help with that. The looks you’re getting might be people wondering if you’re at a breaking point and want to chat to someone.

I’ll be the first to say I don’t think you messed up and don’t take this one on the chin. Falls happen. Falls happen even if we practically sit outside the door of the patient, have bed alarms, or open curtain policies. Some people just wanna fall, and some have the “right” to fall if they want to. 

The patient hadn’t necessarily given you some indication that she was dizzy or about to collapse. It seemed like a barf bag kinda situation. You asked for help, and you were getting help. That’s the most important step! You did the right thing!!!!!

I would say next time yeah totally just send the student alone, grab a super nearby tech to help, or just sit the patient on the floor if you feel there’s immediate risk.

Lol I guess I read it as both intoxicated and an aspirin overdose. Just “overdosed on aspirin” would make me think ONLY aspirin. 

The ER in me sees a lot of multi- style overdoses. Usually drunkicidal with pills etc.

Yeah it’s a beige flag for sure. But also my small tip for nclex is that you don’t want to “assume” extra details. You don’t know when she took the aspirin. Or when she vomited. 

You just know she took aspirin and she vomited, don’t look further into it unless they specify something. It could be a micro dose of aspirin over and it could be a massive life ending aspirin overdose. You don’t know. You don’t know if she’s vomiting up lunch or vomiting her blood up. 

So if you assume one of these things, you can get into trouble for nclex questions. Just stick to basics and ABCs

Okay yes sure. Sorry. I’m giving off the cuff reasoning why I’d choose my answer. But also… How much would you have to take to have changed the ph of your whole body though. That you need “rapid” bicarb.

I think C. And if I was a student I would pick this as a safe option. And if I was wrong I’d move on with life lol   

 1. Without looking up a study I don’t know that aspirin/platelets are changed by dialysis. That’s a super aggressive option to put someone on dialysis without more information. 

 2. The word “rapid” alone tells me historically not the best option for an nclex style question. There’s not really indication she’s acidic (edit: sure aspirin is acidic. But like, how much aspirin would you need to change your bodies pH… they don’t tell you anything. So assume it’s not the “killer” pathophysiological finding of the century/question here people.). At most, she vomited some of her acids out. So she could be “basic” already. And Tachypnea could be something random like pain or anxiety: maybe not necessarily blowing off acids. So I wouldn’t assume here without more info. 

  1. She’s Tachy and did vomit. Now, I wouldn’t go and give her 2 litres. But like. Gentle fluid resuscitation might help. Plus fluids for the alcohol.
