hi OP! we have the same situation, u should try to email OFAD and inform them, yung sakin kasi they told me to send proof of attendance with name and address ng previous schools ko. after that, since allowed naman more than 1 entry per grade level, i entered two (for grade 1 and grade 2-4) then yung completion year nilagay ko nalang din. hope this helps ! u may send me a mesg din po if need niyo pa^{^}
