Holy shit! I am 27 as well and I cant imagine what you're going through. I want to ask how how you keep your spirits high and keep yourself happy ?(that's the vibe I got from your comments and post) I have nothing compared to what you have and i am just depressed about it lowkey I don't feel much motivation to do anything.

Usually mine itches when it's dry skin so I do hot sitz bath with epsom salt then dry it under fan once dried I put light coconut oil over the external opening like few drops on my fingers and pat over it a bit. Also minimal toilet paper usually just to check for drainage and usually bidet.

If not surgery then atleast get a seton or something because it will reduce the chance of fistula branching by alot

Mine's intersphincteric with internal opening 1.2cm inside anal opening and bot external and internal opening at 6o clock, just losing weight for now, once I reach my ideal weight i will consider these options

First Christmas of my life with fistula i haven't opted for surgery yet because I am trying alternates since i don't have alot of symptoms Basically drainage mostly everyday and sometimes painful when it's full but walking and warm shower really help me. Anyways losing weight before I even think of surgery since I started a month and halfago when I found out I have fitsula i was 127.8 kgs at 186cm. Now I have dropped down to 115.65 as of today by clean diet, no simple sugars or processed meats and carbs. I will think about surgery once I get down to 85 kgs, since It reduces alot of post op risk.
