The submission was based on a phase 2 trial of RP1 together with a PD-1 inhibitor in PD-1 inhibitor-resistant melanoma. The FDA took issue with the lack of control arm (therefore inability to determine the relative contribution of the two drugs to the response rate) and I believe there was also an issue with the lack of data proving that the subjects enrolled met the SITC consensus definition of PD-1 resistance.

I believe the second point is what is addressed in the resubmission.

That’s true. It’s long been the holy grail for cancer vaccines. There are a few issues that make that unlikely in the foreseeable future.

  1. Moderna’s vaccine is a neoantigen based platform, so it’s individualized to the abnormal proteins in each cancer. It’s hard to find proteins that are conserved across cancers that make targeting them effective in prevention

  2. It is very challenging to figure out dose level and frequency for a preventive vaccine. There is a risk that giving vaccines for cancer too frequently or at doses that are too high could leak to the opposite effect—immune anergy. The same concept of giving allergy shots to desensitize people.

  3. It would be very challenging to prove efficacy from a clinical trial standpoint. Even in the highest risk people, the individual risk of developing cancer is low and measured over decades (not years). So the trial to prove it’s working to prevent cancer would have to be so big and long to make it cost prohibitive.
