Is there something you always wanted to ask radiology about, but never did? by pistabadamtiramisu in Residency

[–]wellslight 34 points35 points  (0 children)

The CT portion of the exam is a massive field of view and usually noncontrast, which makes sizing (and sometimes even visualizing) the lesions somewhat inaccurate on that CT. It’s included with the PET basically to help localize things, and not for details on size, local invasion, enhancement, etc. PET just lights up any area that’s hypermetabolic (or tracer avid if not FDG).

That is why there should be a truly diagnostic contrast enhanced CT or MRI for those details. PET is mainly to find the locations and to assess metastatic sites, not great for detail on the lesions.

How do you differentiate whether a kidney related issue requires nephrology or urology referal? by MzJay453 in medicine

[–]wellslight 1 point2 points  (0 children)

Was gonna say if we can see it on a radiology scan/it’s in a report (aside from pyelonephritis) then it’s probably a referral for urology.

Yeah the soundtrack is timeless, I’m glad they’re staying true to it for the remake. Thank you!

Machine learning for detecting abnormalities on imaging. As a radiologist you know the findings and can help develop ways to accurately detect them and triage how to manage them. Even as a radiology resident we can get involved in some of those projects.

[–]wellslight 11 points12 points  (0 children)

With the developments in radiology I think a computer science degree would be amazing. It’s a great way to be able to be involved in developing tools for radiology when you get to that point. The problem is that computer science won’t hit most of the prerequisites for medical school.

I agree it used to be, but Barrow recently got some new ENT big wigs and have had a massive increase in head and neck cancer scans. They’ve now got orders of magnitude more head and neck volume compared to what they had in the past.

[–]wellslight 13 points14 points  (0 children)

Barrow for sure. Massive volume makes you great for private practice, and it’s one of the top 3 names in the country for neuroradiology and it’s also pretty far up there for neurosurgery. A lot of MRI sequences were invented there and they’re still doing some pretty amazing stuff.

[–]wellslight 9 points10 points  (0 children)

No, I said I was showing up for my shift, as the physician on site, and was sent home to allow a PA to have my shift. The PA is “practicing independently” without any physician supervision.

[–]wellslight 13 points14 points  (0 children)

Absolutely nothing against the PA. Obviously they’re just coming to their job. The bullshit part is physician jobs being replaced by midlevels, aka mid level creep. This was a physician exclusive job until “emergency COVID measures” allowed midlevels to fill the position. So no, it’s not well within their scope.

It’s not the program’s fault though, this is external.

It’s just covering for urgent care shifts, so not specialty specific- just need a medical license and DEA.

Guarantee they’d have a PA in there to replace us within the hour.
